@@VeerSingh-mu5if If you're in medical field (that's why you're watching this video in the first place) you are already fluent in English because you can't pass the entrance exam without English or survive in med college without English because it's the mode of teaching and learning since all resources are in English. You can't expect everyone to speak Hindi though. I'm from coastal Karnataka, my mother tongue in Tulu. But I have learnt both kannada and Hindi in school along with English and occasional french. So I'm fluent in all these languages because all these are foreign for me. But it's not common for everyone from the south or north east to know Hindi because it's unnecessary since English is the administrative language.
Understanding everything in full English is not the only thing that is needed to know the mother language of India, what is Hindi to know, I have to prepare for NEET also in Hindi.

Also, the the original commentor, I recommend you watching English channels. They usually have easier to understand explanation on the go
