I am a Spanish G. P. Thanks for show us not only a guideline but also the way to cope with these patients, sharing with them your decisions, and putting their lifestyle in the focus in the moment of selecting their treatment.

This shows the absolute futility of managing a lifestyle induced disease with medications and referrals to dieticians. The physician's solution is not to support her to change her behaviours, meal timing, etc but rather to inject her which she does not prefer. This means less compliance and more complex medication administration. Focus on her education and support her to eat and exercise.

i think the better option was that she should have been started on SGLT2 inhibiotrs ie empagliflozin ,or kanagliflozin... as is oral medication once a day,, lose weight though at the expense of likly UTI at times and A1c would not be lowered so quickly as with dulaglutide,. added with sglt2 long term benefits of cvs protection.. along with diet ,exercise ,life style mangment..

Cut out sugar do not eat processed foods, eat natural food )vegetables, vegetables, vegetables) , lower intake of processed carbs, start intermittent fasting. That should allow you to lower blood sugar level and avoid insulin. Look up dr. Jason Fung at youtube,

Feeling really icky about the cliche/stereotype of the hispanic patient, the ‘culturally embedded’ patient-blaming and the overall condescension of the clinician here, even if the clincal info is sound
