I wonder if this was a backyard motor rebuild. Just like the entire truck. It’s has good bones, but she really needs love and stacks of cash. Good luck with that one.
My vote is the C-16. I owned 2 of them. The first one was first Gen and the only time it laid down on me in 900-K was at 290-K it puked a head gasket. And Denver Cat put the upgraded head gasket on it, and that was the end of that. My 2nd one was in my heavy haul truck I ordered. That was my 2003 W900L C-16 18spd with 2 spd rears 3:36/4:33 with a 30-K front axle and a 30-K lift axle in the middle of the truck mounted at the back of the 86" Studio sleeper. I used it from start to finish (6-2003 to 4-2013) to pull my 9 axle 50 ton RGN. And my 30 ton stretchable 4 axle step deck. I put 740-K on that truck in 10 yrs. And other than needing the Jake brakes rebuilt at 700-K. The inside of that engine never had been touched. It had a hot tune Cat program in it that was making 785 hp to the ground. And was getting 5.85 mpg dead heading. Light weight was 52-K and change, full of fuel. And my personal stuff in the truck to stay out a month. Get with Michael at Crome Depot in Mantua, Ohio for all your exhaust and accessories for this rebuild. Her husband has done several of these like you have here. Definitely the C-16. Yoy won't regret it.
It's a diamond in the rough, hell it don't look bad considering the crap I use to get stuck with back in the day when I started driving coast to coast.

Man I had a 03 long n tall. Be sure and check the wiring between the cab and sleeper....behind the passenger side panels.....the unibilts' are notorious for that

I know I'm coming late to this Rodeo...but you should try to do a complete rebuild on just what you can swap,trade, get given,or find in your own collection of parts or even go get yet another truck that has been wrecked or abandoned with alot of the parts you need, engine ,shifter, stacks ,... And if it was me I wouldn't worry about everything being all looking Brand New, try a build that complies with dot ,but is like ratt rod mental...🙂🙏
first thing I always do before tuching anything , wash and cleaning the hell out of it so you really see what you dealing with and what you had to do. I'd get it running and fix nessesary stuff and do a makeover in the next winter.
If there were ever a truck to put that 12V71 you’ve got in with twin turbos like you’re wanting, that 379 is it. You’ll have No packaging issues with the turbos, exhaust and outlet to intake pipes with that long hood.
PDI manifolds are 3 piece . Great system . You can get new fenders from 4 states in unpainted polished aluminum or fiberglass low ride gangster fenders from Thompson's . Truck has " flex air ' rear suspension smoothest ride out there . Usually 3800# load limit works well with disc brakes . That's a Holland fw55 fifth wheel ( big boy )
Its gonna be sweet when yall get it finished, thats for sure. I like the horns myself but only on certain trucks. Holler if you wanna sell them horns if there still in descent shape.

i have lots of buddys who are cattle/bull haulers and let me tell you they run them sunsabitches like they stole the damn thing but goly they look damn nice going down the road

It will be nice to see this one come alive! 😎

Go with jw speaker replacement headlights get heated version and lifetime warranty u will not be disappointed the only light that is good in those housing
Build that truck, everything it needs or is missing is just normal maintenance stuff stuff you would change on an old truck anyway, twenty bucks says you can save that engine also, weld that hole up, probably just a broken rod and most of the time the rod will stay as one piece around the crank, protecting the crank

put a motor in it and fix what u need to in order to safety put it on the road .Run it as is and start making some money and fix it a little at a time . you already have a few projects to finish . that’s what i would do
Got to fix the ol girl up for sure.id do 6nz with Pittsburgh power or pdi tune and swap that turbo and manifold to the 6nz and put bigger injectors in it and a 3408 cam with 6 or 7inch miter cut or flat top stacks.and keep the old style round head lights for sure just put the newer leds in them

If it was going to be turned into a show truck//Every day worker it would be cool to see it with a C18 making at most 1000+ horsepower , but otherwise a 6NZ would be good cool as well

I have a feeling the broken passenger side wiper is broken by closing the hood on it pulled away from the glass. Ask how I know :(

I'd keep the stack you can cut it down and use it as a cheap rig up stack for a beater rig the low elbow stacks work the best for that

I'd put the 2WS in it over a 6NZ any day....helical gears are better than straight cut

The cost of a inframe is a lot cheaper than a new truck if I get a warranty on motor it's worth fixing or replacement

You should put air bags in the front. I think all trucks need to be air ride, I don’t know how the old school drivers survived before air suspension.

As long as the crankshaft is in good shape. If not junk it.
