His holiness is a title for 14th dalai lama. Not for every lama.

As His Holiness the Sakya Gongma Rinpoche is the head of the Sakya lineage, we should use the title of His Holiness.

It is generic English title for all major Lama who are the head of each sects. First portrait of top seven Lama in 1960 includes all the head of sect two tutor of HH Dalai Lama. Of seven only two remained.
