Their PC is running the game at as high of a framerate as it can handle (probably over 800 FPS if it's a decent gaming PC) because crappy games like this often forget to cap the framerate. That's what "Vsync" does, caps the framerate in line with your monitor's refresh rate, but good games will generally just cap FPS at 60 or 120 anyways. This is incredibly common in poorly developed "indie" games, and is WAY more likely than having a cryptocurrency miner. I've experienced it multiple times myself, but I was able to fix it with Nvidia's control panel. But it's really the game's fault. Your first thought when a crappy quality game is making your fans run at max speed should always be "The framerate is uncapped"
Uncapped frames isn't necessarily bad tho. Cause having more frames than the refresh rate will allow the frame it detects to be more accurate and thus be smoother

"Optimized" is a strong umbrella term here, the problem is they didn't cap the framerate to 60 or so with Vsync. Sure it could be poorly optimized, but that wouldn't make a gaming PC run the fans so high. If a really low quality game makes your fans run full speed, your first assumption should be it has an uncapped framerate and is running as high as your computer will allow. This can actually be fixed manually with Nvidia's control panel, not sure about other graphics cards though.

Its most likely the designers didnt understand optimization and the game is rendering everything all the time

According to other comments, the monster appears if you deplete your stamina. Arin is making the monster appear and doesn’t even realize it
