How and when to initiate and titrate Premixed insulin ?

    Nice lecture..
    Sir can u make more vedio like this with real case scenario... Like putting patient on insulin along with antidiabetic drugs and then remove insulin . When sugar of patient is turn Normal.

    Sir you said to start either basal or premixed once daily during dinner time or breakfast...how to manage other meal sugars? Do we add OAD's to cover rest of the meals?
    Example:
    Mr Y was on Glimy 3 forte in the morning & Glimy 2 forte in the night and Dapa-M in the afternoon.
    You started basal or premixed at dinner replacing glimy 2 forte..what about morning & lunch OADs ...should we continue same dose or alter the dose as we have started insulin?

      Other meals can be covered with oad like acarbose or dpp iv inhibitors which work on post meal sugaes

    I appreciate this wonderful session, sir. I have a doubt, but it might be silly 😅. Are there any kind of protocol or guideline for when to stop taking insulin after achieving satisfactory glycemic control with both insulin and oral medications in type 2 DM? Or do we have to take insulin forever once we start? Thank you in advance.

    Sir, what about OHAs with insulin initiation ?
    We should stop sulfonylureas and pioglitazone?

      Need to reduce the dose of SU. Stopping Pio is not necessary

    Can we switch back to only OHAs after achieving adequate HbA1C control with augmented insulin therapy with premix insulin -- sir?

    dose of insulin varies from person to person and thats why one formula does not fits all thats what i feel
