If the company has been of a different country like Europe then they deal directly with the government who contracts out the productions with a binding contract stating the maximum price it must sell for over the period of the contract along with the exact ingredients in it and the quality of product and minimum number of units it has to deliver each month or year. America just allow the company to produce and sell the product at their will with rights on the product so no other companies can produce the same drug, on top of this they are allowed to alter the ingredient and renew their patent so that once the original patent for the drug has passed other companies still can't produce it because it now falls under the new patent, the change can be as tiny as slight change in composition of the inactive ingredients (ingredient that does nothing, only makes the pill large enough to handle because active ingredient is only few milligrams, or in this case EpiPen only has 0.3mg Epinephrine the rest of the liquids in there is just water or preservatives).
