Ang Nurse's notes halos pare parehos lang yung steps. Condition or ano ang ginagawa ng pasyente nung nereceive mo. Neurological status, bedside care/nursing intervention na ginawa mo, medications na binigay mo, subjective and objective observation. Of course yung food intake, consumed all, 1/2 or 1/4 of the served food, all intake and output. If you are a nurse, no need to ask how to do charting? Kada hospital may format pero pare pareho lang yan. Diyos me, no need to explain. Continous and ongoing manner ang documentation. Document everything you did and observed, including patient's concern. If it's not documented, not done, ang saying na ito ay alam ng lahat ng nurses, kahit student nurse alam ito. Ang tanong ko, anong language ang ginagamit sa pag charting? English or Finnish?
You project yourself as smart (maybe too smart for your own good). Ang gusto lng ni maam Julie ay mkatulong sa mga baguhan na nurses sa mga care home sa Finland to make the job easier for them in regard sa gnagamit na vocabulary in Finnish (actually, this in itself already is a ready made charting in Finnish). Maling simula mo. Ngmamagaling ka. Try and start again. This time, write and ask nicely if you plan to work in Finland. PS. Walang salitang continuos sa English kundi "continuous" ang tamang na pag sulat. Maybe this answers whether you are "magaling" as you project yourself to be.😂

So, bakit ituturo pa niya kung ano dapat isulat sa chart, kung nurse ka siyempre alam mo yan. So, sino tinuturoan niya?

She was not informing about how to chart. That is a given. She was informing new nurses working in care homes POINTERS how to write their charts in FINNISH so that they don't have to worry on this part of the work. Dhil mahirap na salita ang Finnish language, it is a hard part in their performance of their work so these pointers would help very much. At saka hwag mong iimpose or assume na yung alam mo na sa charting ay iyun na because besides the standard charting as we know, maybe the agency that runs the care home may have their own specific instructions how to chart or what to chart for. And hwag kang maangas dhil baka wala kang bnatbat sa skills and nursing in general to those who are interested in the pointers that was given by Julie. Understand?
