    Hi Dr Lee. I have been binge watching your videos for the last 2 weeks and I was super scared about taking my skills state exam. I took it yesterday and I just found out today that I passed my written and my skills. My three skills were counting and measuring respirations, changing an occupied bed, and side-lying position . If I didn't watch your videos I know for certain that I would have failed. Thank you so for making these videos.

      Hello, Prometric does not require gloves for the occupied bed skill. Just an FYI, Prometric will start testing oral skills in December 2022.

      Hello Yesenia, Prometric only requires the student to change three things ( top sheet, bottom sheet, and the pillowcase). You don't need a privacy blanket if you can keep the client covered with the top sheet. A pad under the client is not required and will not affect your score.

    Dr.Lee is it okay if we turn the patient away from us when doing this skill? I was told we could not have the patient facing away from us and we would get points deducted if we did

      Hello Daryanna,
      You can turn the patient towards any side for this skill and you will NOT get points deducted. Prometric does not tell us what side to turn the patient to. See the instructions below. Pay close attention to steps 4-8.
      Does the candidate:
      1 Greet resident, address by name and introduce self? (IC)
      2 Provide explanations to resident about care before beginning and during care? (IC)
      3 Keep resident positioned a safe distance from the edge of the bed at all times?
      4 Remove and replace bottom sheet on one side of the bed, before turning resident to remove and replace sheet on the other side of the bed?
      5 Keep resident positioned on a bottom sheet throughout procedure?
      6 Secure bottom sheet to mattress (e.g., for fitted sheet secure over all four corners of the mattress; for flat sheet, tuck at head of bed and on sides and extend toward bottom of mattress so that resident’s heels are not against any exposed mattress)?
      7 Leave bottom sheet free of creases or folds?
      8 Turn or position resident to remove or replace sheet(s) without pulling sheets in a manner that creates friction and risks skin shearing?
      9 Replace the top sheet over resident with a clean sheet?
      10 Tuck top sheet under foot of mattress leaving sheet placed loosely, avoiding pressure against toes and allowing for foot movement?
      11 Leave top sheet placed on top of resident to cover body up to shoulder level, without tucking in along sides?
      12 Keep pillow positioned under resident’s head throughout and at the end of the procedure, except when removed briefly to replace pillowcase?

      Hello Abolanle, Gloves are NOt required when making an occupied bed during the state exam. During the state exam, you will be making a bed with your partner as the patient. The bed linen will be clean for the most part, therefore gloves are not needed. The evaluator will be grading you on keeping the patient safe, away from the edge of the bed, covered at all times, and making sure the client does not lie on the mattress.

      Hello Aiko, The clean linen will touch the dirty linen when making an occupied bed. The patient CAN NOT touch the bare mattress. No, you don't have to leave the patient in the middle of making the bed to wash your hands because you will always have both (dirty and clean) linens on the bed until the end. Does this make sense?

      You do not need to place a drawsheet on the bed. Prometric is very clear with stating what they want you to change for this skill. Only do what the STATE is asking you to do. A drawsheet is never mentioned.

      You will be just fine removing the top sheet and covering the patient with a blanket. The main thing here is to keep the patient covered for privacy.

      Hello Christine, Thank you for asking these questions.
      1. You can ask the client to move over in bed. We are not expected to lift the person when the client can do it for him/herself.
      2. Yes, it is a very good idea to ask the client to hold the top sheet while removing the bath blanket. This will help to ensure the client stays covered which is graded under indirect care skills.

      Hello Relauna, I would study all of the skills except the oral ones because they are not being tested due to COVID 19. Handwashing and indirect care are skills that everyone testing will get. You will not know the skills you will have until the evaluator tells you in the testing rom.

    Hi Dr. Lee! I'm struggling a bit with this skill. I keep going over the time limit because I find it hard to shift the person from one fitted sheet to another. I move them like you suggest in the video, but a lot of the sheet material ends up stuck under them and I feel like I have to really yank to pull the fitted sheet to the other side. Is it bad if I yank? I hold the resident to help them balance and keep them centered in the bed. Do you have any tips you can offer? I really appreciate all you do!! I'm testing on 6/30/21 and you and Nurse JAR are really seeing me through this!

      Hello Cherriesllol, When changing the bed linen, place the dirty sheet next to the person's back while they are lying lateral. Then, when you get the clean sheet, tuck it under the dirty sheet that is next to the person's back. You can also ask the person to tilt a little so that you can tuck the sheet under the person good. When you ask the client to turn to the opposite side you should she the clean sheet and not have have problems pulling it out.
      Practice at home making your bed with an adult and see if it will work. take care.

      Hello Julie, Prometric does not REQUIRE us to check the bed wheels when performing this skill. The bed will already be low and the wheels are already locked before your partner get into the bed.

      Hello Hyacinth, Yes the dirty sheet can touch the clean sheet. The important things here are the patient stays covered, away from the edge of the bed, and never lays on the mattress.

      Hello Jared,
      When making an occupied bed you can use the client's top sheet to keep the person covered or bring a blanket over to the patient. If you bring a blanket to the client, I would suggest placing the blanket before you start changing the bottom sheet.

      Hello Obianuju, I plan on doing the oral care skills soon. The reason I haven't done them so far is that Prometric will not test on these skill during the COVID 19 epidemic. Everyone will be required to keep their mask on during the state exam. Things may change in 2021 after more people have received the vaccine for COVID 19 and Governor Abbott gives an update on our status in Texas.

      Hello, Please read the Prometric directions. Gloves are not required to change the bed. What step did you see gloves as a requirement? If the state wants us to use gloves, it will be listed as a step. See the steps for skills with perineal care and catheter care.
      Gloves are not mentioned for this skill. Recheck the requirements.

      Hello , The only requirement is that the patient stays covered while you are making the bed. Some people use the top sheet and others use a blanket. Here are the guidelines under patient rights in the Indirect Care Document for this skill.
      close privacy curtain before providing care
      refrain from leaving resident completely uncovered while top sheet being replaced
