Jake was killed in a case of false information, when johnny allen had his heart attack in prison shortly after sean slater visited him, someone runs to johnny and johnny keeps saying get jake moon, the guy thought johnny wanted jake moon dead but he was going telling him to get jake moon and tell him to come see him but he dies when he says get jake moon, you don't see his death because he's killed off screen
We don't have conformation. His status is "Unknown but likely deceased" but no more than that. Until official conformation has been made, his ultimate fate remains a mystery
