Emergency Department Services: New Problem with Additional Work-Up- 2021 E&M and Beyond

  • We’re halfway through 2022 and already we're looking forward to the end of the year to see what those 2023 documentation guideline changes will be. Personally, I think I'm most excited to see the expansion drift into the emergency department services specifically. Where we see most discrepancies occur is at the point of New Problem with Additional Work Up -and this being the biggest differentiation between that 99284 and the 99285. So, let’s step out of the box for just a moment and discuss New Problem with Additional Work Up in the ER.

    HI Shannon - Doesn't the ED have to provide the "additional workup planned" there at the ED (e.g. ordering labs, tests, imaging), rather than stating the patient needs additional workup elsewhere or by a separate specialty, to count for the the 4 points? I was under the impression "additional workup planned" is the provider ordering labs, tests, imaging, but they have not received the results back during the DOS of the encounter. -Thanks!

      GREAT QUESTION! Remember additional workup is part of Marshfield Guidelines, NOT part of 1995/1997, and there are NO rules or guidelines for the intentions of Marshfield. Therefore, that really is up to individual interpretation. Additional Workup- the words infer it is beyond todays encounter, and as stated in the video, that is how many MACs had interpreted this when guidance appeared on their website.
