A sword becomes permanently black for reasons still unknown, but it has been theorized that it happens when you bestow part of your life into it together with your haki. Like you give, let's say, 20% of your lifespan and haki pool forever. Again, just theories. Also, Yoru and Shusui are the only 2 black blades ever confirmed.

No... Kokuto and meito are different properties. Being a kokuto doesn't make a blade saijo o wazamono. Sushui is still an o wazamono grade, formerly ryo wazamono. Murakumogiri is not a kokuto but is a saijo o wazamono.
