Transocean crew was in so much pressure to finish that they forego safety procedures and BP approved of whatever they are doing as long as the procedure can finish as soon as possible. It bares a lot of similarities with the Chernobyl incident where a common procedure was being done but the higher ups want to test the limits. Basically a "f*ck around, find out" event but with machineries and corporate suits.
