check the pages. make sure that you have dashboard page created.
how does one get notified on who requested to be an affiliate and how does the affiliate get notified that he/she has been accepted
it will send email notifications. Look for email settings of the plugin.
Do you know if there is an option to add promo code
do you mean coupon code? yes, you can add coupons.

Meant the referral link on Mobile view
do you get link on desktop view.

It is causing fatal error to the website
may be conflict with another plugin. which theme do you have?
