      Dear Dr. YOU have been Such a HELP.
      I am a mess. Had r knee meniscus operation 2010 l hip replacement 2014
      r hip replacement 2016 always doing workout to have strong muscles. Now this Spinal Stenosis is SO painful.
      Have to get my MD to know my Orthopedic to know my chiropractor to know my Spine Dr.( found my SS with MRI in 2010). Arthritis IS a challenge. YOU are a BLESSING explaining what is happening.

      What is the best cure for the nerve pain from the hip to the thigh then front part of the leg. I had a painful spark in my lower back which is now causing leg pain. I'm tired of pain killers.

    If I ever needed a neurosurgeon again, I would do whatever it takes to have Dr. Su be my physician. His keen ability to clearly articulate complex phenomenon and his demeanor are evidence of his vast knowledge, expertise and empathy. His approach indicates he carefully listens to his patients' concerns for a thorough assessment, diagnosis and thoughtful recommendations. I fractured L1 and another neurosurgeon performed a kyphoplasty within 3 days of my injury without informing me of the risk, benefits or alternatives. Although I am a Dr. myself (with a different speciality and skill set) this was a troublesome experience and, frankly, I wasn't thinking clearly. I merely wanted rapid pain relief and erroneously had faith in this neurosurgeon.The outcome was poor. She never made arrangements for a follow-up. She could learn considerably from Dr. Su as her mentor. I rightfully acknowledge admiration of Dr. Su. He is an exceptional role model and much appreciated.

    Thank you thank you I know that I'm really late but I just found your video, for the past year I have been getting CT scans MRIS going to various doctors and not getting an answer I have had an EMG I've had a lot of things confirmed but not as far as my bone structure. Now I have a PT instructor and I religiously everyday do my PT. So that said I have congenital stenosis wow I wish I would have been told about that one, my family is not communicate very well. I Now understand a little bit of why my left side of my body is not as strong as the right side no matter what I do and why I can only walk 100 yards and sit down and feel better you're the first one to tell me that thank you so much

      I started out about a year ago with very stiff lower back. This turned into hip pain which now runs through my hip into my leg (left) and often down the front of my shin into the top of my foot. Sitting too much causes an increase in the pain so I spend as much time standing as possible. MRI shows a myriad of issues but this pain through my hip and deep buttock to left leg can get unbearable. Leaning over slightly and reaching down causes increasing pain until I can’t straighten back up without stretching up to hold the top of a door then crossing bent left leg in front of front right leg then reversing that motion. Very painful on first movement but immediate relief after that. Same with putting my interlocked hands behind my head and physically lifting my spine up straight while tilting my pelvis up, very painful at the very start but instant pain relief after that. Chiropractor say my hip SI joints and hip joints are tight but trying to do stretching physical therapy excercises creates that leg and hip pain. I am 66 and have been thrown from horses landing on my tailbone a few times in my younger days

    Yaaay for having congenital stenosis! Tried all sorts of medication to no avail and can confirm the shopping cart thing is legit. Walking normally is rough. Although sitting does help, I find I need to alternate between sitting and standing as the pain is constant. I love being old at 27 🤣 It has now progressed to the point where I have numbness on the right side of my lower back, and from my knee down. The pain has spread into my left side of my body as well and I can barely walk as it gets too painful and my leg gives out. I need to use a cane at almost 29 years old and can't even walk around outside. I live in a basement, wasting away, with no money or financial support from the government, mainly because I'm "too young" and "haven't tried everything". I've been waiting over a year now, just to see a neurosurgeon. Meanwhile, my physical, and mental health have been rapidly declining, due to feeling hopeless that nobody will help me and I can't afford anything. I'm $50,000 in debt. I wouldn't wish this upon my worst enemy.

    Thank You very much Dr. This video was indeed helpful to me. I have always been going for an xray scan and the results always say I'm fine whilst within me I still feel much pain, tingling and numbness throughout one of my legs to my lower back right from my adolescent stage to my age now at 27. Now I know that what I need to go for is an MRI when i have the financial capacity. But at least for the mean time, there is this relief of understanding what I am going through. Thank You once again Dr.

    Been there. Had a laminectomy in 2007. Lasted under 4 years then needed a L4 L5 fusion for slippage.

    My advanced lumbar spinal stenosis was seen ( along with large bone spurs, stage 4 DDD & SI joint issue ) on x-ray 24 years after 1st dx. from MRI.

    Very knowledgeable and easy to listen to. I’m 39 now, broke two vertebrae L3L4 when I was 18 and three years ago my right knee went numb, couple months later full sciatica down my back right leg and shocking sensations began, 200 plus a day in several locations throughout my entire foot ankle and toes. Leg shakes and has cold spots. Every surgeon just says emg looks ok and no compression on S1 nerve no need for surgery. They assume my back hurts because my vertebrate look compressed in the MRI but it’s a completely different story when it’s my leg.

    Thank you for sharing this, I have been dealing with this for the last couple years and recently had an MRI that describes me as having most of these items that you talk about. When I meet with her i will at least have an idea of what she will be talking about.
    L4-5 disc bulge, spondylosis, spurring, severe facet degeneration,
    degenerative anterolisthesis. Severe central canal and subarticular
    recess narrowing with complete obliteration of spinal canal. Mild to
    moderate bilateral foramen narrowing. Possible synovial cyst in the
    left subarticular recess.
    ( I have not yet met with my Physician to discuss)

    Thank you for your reply Dr. Su. I am schedule to see a Pain Management Physician this week - the Orthopedic surgeon indicates I am not a candidate for surgery. My 2020 MRI results indicate L3-L4: Moderate facet degeneration has increased; L4-L5: Disc bulging has increased, Left facet degeneration has increased, Moderate right-sided neural foraminal narrowing has increased; L5-S1: Mild disc bulge with facet degeneration causing moderate to severe narrowing of both neural foramina, increased. IMP: Moderate degenerative changes of the lumbar spine has progressed.

Everything else I’ve read tends to say surgery is relatively unsuccessful, but you seem to indicate it has a good success rate.

    I wish when you’re diagnosed with spinal stenosis from your care doctors rice, rightist doctors that they would explain my way did you have you include everything from what spinal stenosis is to medication and you discussed surgery rate I do have spinal stenosis

    It is painful and inhibits normal body movements like bending, twisting and makes sleeping differcult.
    The suffering person is often, grumpy,negative & and gets depressed easily due to a lack of sleep and constant aching pain.

    Woot woot for having lumbar stenosis at age 12 and surgery at 15! Then at age 27. Now 32 and just had an mri done and I have it again.

      And the Dr. at USC SAID NO MORE SURGERY!!! The percentage goes way down to almost nothing. I'm just suppose to live like this for the rest of my life. I hope you get cured from all your pain cause I know what the number 10 on a pain scale is. God Bless you.

      Now my back is so tightly close I can't get a pain pump.i had one of those for 12 years in which case it had to come out for my last surgery and now they are saying they can't replace it?

    I was told I have spinal stenosis years ago after getting a M.R.I done my pain really comes on when I sit or try to sleep so I have been using a heating pad for more than a decade nothing else helps the pain better than a very hot heating pad on my lower back and butt, naproxen helps as well, I can't drive very long without being in pain not sure what the fix is I will be in between jobs soon since everything is slowing down at work.

    Very informative thank you, I have spinal stenosis L4 L5 with horrible neurogenic Claudication and pain, pain from my left butt cheek down the back of my left leg into my calf , the worst pain is below the butt cheek my legs feel tired and weak, hard to do my job, because of it all I'm limping after 3 hours of being on my feet and working.

      It sounds like you may be a good candidate for surgical decompression if non-operative methods fail. As a disclaimer you should always consult your spine surgeon

    Hi Dr Su , I'm 69 years old and had a decompression of 4 & 5 in February this year as an emergency all was great and had 3 months of work as a plumbing & heating engineer. After returning to work 2months later I started getting the same problems in my right leg from my bottom to my knee,identical back to my doctor and they sent me for a MRI now I'm been told that 2 / 3 / & 4 are showing signs of the same problem. Is this a common factor. Great video many thanks

    Thank you! Such an informative video. None of my Doctors have sat down with me and explained what these medical terms meant all I got from my primary was "This just means wear and tear. I can send you to a surgeon and they can dig it out." I have since changed my primary care. I've done the shopping cart thing it's great until I need to stand erect. I can barely get back into a normal position why is this?

      When people have lumbar stenosis they typically like to be in a flex posture. There are quite a few non-surgical and surgical treatments on my videos. As a disclaimer please consult your spine surgeon

    For an individual with symptomatic spinal/foraminal/lateral recess stenosis, what would explain a progression of symptoms where there is no relief with rest or sitting? Or, when pain is not present and the individual can take long walks but it is always followed by pain and disability lasting hours/days?

      Groin pain can sometimes be referred from the disc itself. You can always have a diagnostic injection into the hip to rule out the hip as a cause of the pain. As a disclaimer you should always discuss this with your surgeon.

    I have cervical stenosis on C4 C5 and C6, does this cause numbness and a little tingling on front sole of both feet which i have, or could it be the

      Cervical spine stenosis can cause tingling in the feet. As a disclaimer please consult your spine surgeon.

    Thank you for this very informative video! Question: can stenosis cause weakness in the hips? I've got several problems in the lumbar spine and am to the point where I keep getting the weakness & sensation that my hips(& lower back) might give out... like I may fall or crumble with no control. What vertebrae would cause that?

      Yes compression of The l4 and l5 nerves can cause weakness today as well you should see a surgeon

The VA moves a bit slow- I fell two years ago because I couldn't move my left leg(broke my ankle in three places) and I told them it was because I couldn't move it like there was no signal to it. They just now are sending me through physical therapy for it. I think they are waiting to see if the weakness gets better or worse with PT even though I've told them that it is more than just the leg and includes the feeling like I could fall/ collapse from the hips/waist.

      @Dr. Brian Su - The Spine Guy So the VA finally did an MRI. Severe disc bulge at L2-l3 causing Redundant nerve root syndrome. I wish I could post a pic, it's crazy looking. An EMG shows L5-S1 nerve damage that is starting to heal. Are there specific surgical recommendations for Redundant nerve root syndrome or is it to treat the stenosis. How often do the nerve roots return to the normal position?

    Dr Su, do you accept patients from Canada? I have been diagnosed with 4 compressed nerves that are affecting my buttock, hip, leg and feet. I also have significant arthritis in my lower back. Two separate issues. I do have recent MRI to verify. I have a long history to support my issues. I have had problems with getting a surgeon to do the repairs. Your response will be greatly appreciated.

      It is gas within the disk space. Usually not painful but can be an indicator of abnormal motion across the disc space or degenerative disc disease

    Hi Doctor Su, thanks for the video. Last week I had a lumbar MRI and the following was noted for the L5-S1 disc: "Posterior central 2.5mm disc protrusion-subligamentous disc herniation extends into the epidural fat and indent the thecal sac. Central canal and neural foramina are patent." In your experience, is a 2.5mm protrusion serious? And, does the term "patent" mean they are not being pressed by the herniation? I DO have bad sciatica symptoms in my right leg and foot (and in the left, but to a lesser extent) , and has been getting worse for the past 4-5 years. Thank you.

      I do not recommend it as a standalone device. It is not meant to hold on its own it is meant to be used along with a cage. There's not enough data now to show it success without the addition of a cage for biomechanical stability.

    Dr. Su I have been dealing with debilitating back pain for years…diagnosed with degenerative disc disease almost 20 years ago w/ spinal stenosis…then I had a car accident 6 years ago where I broke my neck and have 4 herniated discs…what are my options??       You poor sweet woman. I have no solutions as I’m in the same position. I’ve been in excruciating chronic pain for over 10 years now. The only time I’m out of pain is when I’m asleep. I often wake up because of the pain. I know exactly how you feel. Chronic pain changes a person. Their mood, confidence, outlook, personality and my only suggestion is to seek some type of emotional help and support, do you have a family member or friend that can help you through the maze to find medical care, therapy and help with any social services in your area? It’s daunting I know, but if you had help to start a navigation it might remove some of the stress and feelings of being overwhelmed. You must remain hopeful, no matter how hard a day has been. Keep seeking ideas and therapies, especially the cheap and free ones. I found a video that talked of natural path remedie to decrease and dissolve stenosis bone Spurs. It’s cheap and accessible from Amazon. The same remedy was suggested by my own naturopath. It also apparently works on horses bone Spurs so I’m hopeful. Simple mineral supple to, nothing crazy. It can take 3-6 months to see any results but many reviews from different sources have had hopeful results. Start investigating HYLAND’S CALC FLUOR #1 6X and HEKLA LAVA 30ch….they are simple minerals so cheap and very safe but showing stellar results for spinal stenosis in particular. ……when u have been going through pain for so long like you and me, you’re willing to try almost anything to restore hope. I’m wishing you the very best of hope and freedom, you deserve it!

      An epidural injection is where the medication goes. Into the epidural space. The medication itself is steroid which is the same as cortisone. In that sense there is no real difference between an epidural steroid injection and cortisone.

    I’ve already had L3 L4 L5 fusion. It’s been just over a year and I’m worse not better.. My last appointment with my Surgeon he concluded that the Nerve damage is permanent and I should learn to live with it!
    I’m considering a 2nd opinion but I’d like your opinion. Any and all suggestions and/or information appreciated greatly.. Thanks

I just got my mri result my L5descending nerve root mild indentation. lL4 exiting nerve root with left intra and extraforaminal component resembling a dumbbell configuration measuring 2.28/0.8cm feature are suggestive of a neurogenic tumour .       That is a sign of lumbar stenosis. It's called neurogenic claudication. As a disclaimer you should contact your spine surgeon.

      Multiple epidurals have been shown to cause osteoporosis in the surrounding bone.

    I've had five radio frequency neurotomy so far in the last two and a half years I had a laminectomy on l 3 L4 and L5 and now I'm still having pain in my quadricep on the front of my leg after I just had number five radio frequency neurotomy can't take these pain pills forever

    Dr Su I use do more activities especially walking I love walking now I can't even walk 5-10 minutes a day when I have to hurry up from walking my dog cuz my back hurts so bad .I'm having a lot of pain for probably 7 years from the middle of my back, it feel like I'm carrying a bag of cement when I walk my dog it hurts is so uncomfortable and now my lower back by the tailbone down my leg my hips both sides of my hips can't sleep having these leg syndrome now I end up hitting my legs so much that I could say eggs so much it hurts I'm crying not sleeping sometimes I don't even sleep for a few days ,Dr. said that he seen on x-ray that a screw is unloose on the bottom by the tailbone somewhere,but I'm having a MRI on 9/1/22 I guess I go from there. My question is if a screw is to lose in the Bottom by the tailbone somewhere is it a an hour surgery that they just do a small incision the bottom and tighten the screw ,but what I think is there's a pinching nerves on both sides of my hips that it hurts very bad it's like somebody just kicked me in the lower back that stops me from walking sleeping in the pain on my both legs. I've been watching a lot of your videos and you're saying something about bone spurs that could be the problem is and I like to see if they could do what you're doing by put this metal in between the bones to stretch it up from not pinching . What kind of surgery should I get? I had confusion surgery in 2011 when I had from L1 to L5. I'm wondeing what to do. Thank you for your video's , I've been watching.

      I do have sciatic pain shooting down my legs and my right leg I notice I can't even fold it in half cuz it hurts so bad shooting pain in there never hurt like that before .I do have arthritis as well comes with age.😰 I just have to wait to go get my MRI and then see a surgeon and see what the Dr. say from there .😢
      I'm hoping for the best and 🙏🙏. I hope they fix whatever is going on. I like how you trim the bones and widen up the space from L1 l5 on your videos. Hopefully the Dr. Can come up with a solution to help me from all this pain I've been suffering, cuz it's very painful hurts so much . I do walk three times a day because of the pain but I push myself because I have to walk my dog and myself for a little exercise ,but at night time I'm stretching my legs and hitting my legs holding my lower back cuz it hurts so much. I just cry and say please stop and I get up and walk up & down the hallway during the night, I do chores like vacuuming ,washing clothes

      The upper lumbar nerves l1 to L3 can often manifest as pain in the hips. You would have to be examined to discern that. Often doing a diagnostic injection into the hip joint or over the upper lumbar nerves will help with the diagnosis.     I have compression fracture With fracture in my L3 and L4 with pich nerve. I was also diagnosed with severe osteoporosis and low bone density. Hips,lower back are aching specially if I cough,sneeze and sudden movement. What's the best treatment for this...non so invasive procedure?is kyphoplasty one of the best. I can not afford fussion and I am not ready with that kind of surgery. Please advice me Doctor. I need to improve and if possible get my life back. I am 48 y.o

    I’m 40 years old and I recently had an xray done that said I have disc space narrowing between the L5-S1. I already was diagnosed with sciatica months before the disc space narrowing symptoms. My main problems are: pain in my leg, mainly calves, hyperreflexia, trouble sitting, standing, or walking too long, and I have symptoms of claudication in my left leg which is painful. I have gait problems that come and go. I’ve been referred to get an mri two times, but my insurance keeps denying it. Will this be permanent or can it get better with treatment?
