@@vermilisix not always, can be off normal rooms, not even always diagonal either. I've had them cardinal to normal rooms before, though, they are more likely to be diagonal.
on the floor with the diplopia shop, couldve used both eden's souls in the secret room and maybe gotten rock bottom, then picked up a red stew or two. would've had more than enough charges from the boss rush and plenty of cards to tp out with the two booster packs. but to be fair i guess you didn't exactly need it LOL
Usually I find ultra secrets diagonal from 2 normal rooms at the edge of the floor, so I think the ultra secret would have been unlocked if you went one room up, don't know if it is definitely true, but found 4 ultra secret rooms in 1 run by trying this way
I know there's probably a good reason why you don't do this. But I think you should take the risk and get the coins that are covered by crowds of enemies. You'll still be able to get some health back as well as the money so in the end I think it's a good trade

They always appear next to 2 red rooms, usually ones leading off from special rooms e.g. treasure rooms and vaults
