    Question:
    #1: Answer D also grammatically correct? See also #14, 16.
    #14: Are there no conditions under which answer C could not also
    be correct? For example, "Yes, you could exercise more, but I'd
    rather you give up smoking."
    #16: Again, why not answer C also?

      For two questions:
      #1: Answer c ("went") is grammatically correct and
      #14:Answer a ("gave up") is grammatically correct when the subjects are different. Here's the rule:
      * When the subject is the same, we use: Subject + would rather ('d rather) + base form of the verb
      ** When the subject is different, we use: Sub 1 + would rather + sub 2 + past tense.
