For "fighting as equal" I would believe you first have to unchain her and be honest with your intention of a fair fight but maybe there is another variation to go along this line.

I think that one extra line where you confirmed that you weren't lying about being there to slay her may have built enough of a rapport with the Princess that the game decided she wasn't expecting the stab. I have never had it confirmed to me that there is an invisible point-based "reputation" system in Chapter 1, but I've long suspected it.
Because he doubted she was dead - so she wasn't dead. Simple as that really. Plus she was also waiting for the right moment to strike back. Plus that dialogue option doesn't really matter - if you tell her on the stairs you're going to kill her, you're more focused on doing it, whereas if you remain silent or say anything else you can only do it if you tighten your grip without talking to her.
Remember, The Princess is what you perceive her to be. If you don’t believe she’s dead, she won’t be.
