I was asked if the hemodynamic tracings of severe PS and PI are similar to the tracings of AS and AI. The PA-RV systolic tracing of PS is somewhat similar to the AS systolic aortic-LV tracing. The diastolic behavior is however different. On the right side, it is easy to have end-diastolic equalization of RV and PA pressures, since the difference is not large normally; RV-PA end-diastolic equalization is frequently seen outside PI, in RV failure, restrictive and constrictive processes, and commonly in PS. Hence, PS has delayed PA systolic upstroke but also equalization of end-diastolic RV and PA.
Therefore, in PI, end-diastolic equalization of PA and RV is not enough for the diagnosis. Nearly full diastolic equalization of RV and PA is seen in diastole in PI. Review the PI tracings under my talk th-cam.com/video/DFbsXsNeqzw/w-d-xo.html.
