  • As a young ER doctor, Peter Attia felt contempt for a patient with diabetes. She was overweight, he thought, and thus responsible for the fact that she needed a foot amputation. But years later, Attia received an unpleasant medical surprise that led him to wonder: is our understanding of diabetes right? Could the precursors to diabetes cause obesity, and not the other way around? A look at how assumptions may be leading us to wage the wrong medical war.

      What are you talking about? The world needs more humans..regadless of the career they chose....to be more humble, genuine, to resist judging others, to have empathy...to recognize that every human as an effect on every other human (the extent of that effect can be argued) and we all have an obligation to one another to do exactly as every mother has told their offspring....'treat others like you want to be treated.' When you say you wish there were more doctors like him, you're forgetting the effect that non-doctors can have on others. Humans are humans...you chose a different career than I did...so what. Doesnt' mean you won't make mistakes...you will...doesn't mean you cannot harm...you can. The doctor in this video is called an adult. 95% of everyone you meet that is over the age of 18 is nothing more than a child in an adult body. It takes 2 things to be an adult. Neither costs a single dollar, but they are HARD. 1. Recognize we all a have an effect on one another...positive or negative. Let that sink in...that means with every decision you make...you must consider how that decision will effect others...positive or negative. 2. If your decision effects others negatively, apologize for it, work to repaair the damage you caused and don't do it again.. Pass the informatoin on to your dependents. That means you have be able and willing to admit you made a mistake. That is why 95% are children in adult bodies.

      kimberlee carter yes maam-i had the same sentiment...wow a doc that doesn't say he has all the answers. I had begun following these guidelines on my own after experimenting with various other ideas and I have not had to use my insulin.

      I don't know anything about diets, but i always found the low carb people weird indeed. Italians for example live on pasta. Thanks for pointing it out and the quick answer!

    This is the best Ted talk I have watched and I lost my mom 3 years ago when she was given the option of amputation and refused to let her legs go. I lost my sister to pancreatic cancer last year. I wish every doctor could watch this and have the same conviction to get to the truth and make a difference to millions of people who have been treated with care for the profit of big pharmaceutical companies and HMO’s that decide care without medical training. Thank you for this. FDA, CDC, and our government has failed in their duty to do what they exist to do.

      I have multiple conditions where eating certain vegetables or foods exacerbates one or many of the conditions. Doctors generalize what you an eat and say you must follow those rules to get better. In fact the rules haven’t helped me at all. I’ve tried so hard to follow them mostly to no avail. I haven’t had the privilege of finding one single doctor who has taken the time to research and explain anything. I do it for myself and slowly have found ways to help myself and have stopped feeling so overwhelmed by this. I do what I can eating pretty healthy, even though the food may affect me adversely because of the different conditions ,( since childhood), but its been a tough road.

    This was 9 years ago, but I feel it’s still very appropriate in 2022, we don’t know what we don’t know! This is a high quality individual that really cares, I so appreciate his words and am looking forward to learning more about his practice. So many people in my family died from the complications of diabetes and my dad followed everything his doctors told him to the letter, he got his weight in control, at the end of his life, it was his kidneys that finally failed, he chose not to do dialysis after giving it a try and realizing his quality of life was going to be terrible. Dialysis one day, sleep the next day, etc. I miss him

      He is still in practice and has a good Podcast, "The Drive". His frank and open discussion of a Doctor's biases and the general medical profession is incredible. My beloved wife died from diabetic complications following the AMA "Standard of Care" practices. We trusted the doctors and followed their advice. We were told that it is a progressive disease and there was nothing we can do but allow the doctors to "manage her care" for the rest of her life. Two years after my wife's death, I discovered that I was also on the diabetic path. I was morbidly obese yet was extremely motivated to lose weight. After losing 45 lbs, I stalled. I diligently searched and discovered that I was insulin resistant and on my way to becoming a Type 2 Diabetic. I was following the ADA guidelines of lose weight and moving more. I learned that my many years of high insulin levels lead to insulin resistance. In all of this time, no doctor ever ran a fasting blood insulin test or understood that High Triglycerides (above 100) and low HDL (below 40 for men and 50 for women) is yet another indication of insulin resistance. This can go UNDIAGNOSED for 10- 15 years before our blood glucose levels rise and become uncontrolled. I have managed to stop the progression of my diabetes by following a very low carbohydrate( under 2o total grams of carbs daily), medium protein, and higher healthy natural fats for over five years. I have shed 180 lbs and keep it off. I help everyone who is willing to hear that there is HOPE and a FUTURE when we are given accurate scientific and health information. You will find the TRUTH when you seek after it with your whole heart. Shalom(Peace)

My twin sister passed away this year even tho she was far too young and in my sadness I’ve let myself fall into a sedentary reclusive lifestyle. I’m 34 and wanna get out of this rut before I do serious and G-d forbid irreversible damage to my body. I’m about a hundred pounds overweight right now.

    Patients can smell a physician's contempt from a mile away. It's why I'd rather take a beating than go to the doctor. Thank you for this video - some of the docs I've been to could learn a lot from you.

      I put Doctors right up with Used Car Salesmen, and one step above lawyers. I type two diabetic, and of late my blood sugar has been all over and on the high side, my eating habits have not changed, I talked to my doctor about it or I should say emailed, they don't talk to you anymore, I been having some numbness in my feet, so no exam, not nothing, take Gabapentin! I can take this stuff, and I can stop doing the work I do, it's not permitted if you are taking this stuff, so I don't. My guess before long they are going to tell me that I have to go on insulin injections, then I am really screwed with my way of making a living!

      Fluffycat6, get yourself a new doctor. I had one that was terrible. I had doctors want to shove me into a hospital and on dialysis. I am now working with a new endocrinologist and so far so good. The one major difference that I have noticed is, that this doctor listens to my concerns. I used to be able to control my glucose levels with diet. Now fasting my glucose levels can be in the 300's. I told him I was concerned that I might have insulin resistance and or my pancraus was not producing enough insulin. He agreed and we are tackling the problem and hopefully I can stay off insuling injections.

      Of Diabetes. > Please consider reading this. My contact is a newly retired but very very distinguished neuroscientist who is appalled at the way things are going even here in Australia where the medical profession is infinitely more in touch with the logic of the medical sciences than in some other countries? > We all know that Banting and McLeod discovered insulin in 1921 and subsequently went on to receive the Nobel Peace Prize for isolating insulin which gets glucose out of the blood, but "blood" is the "weasel-word" here. Insulin doesn't get glucose out of the body; it just gets it out of the blood. And therein lies the problem. For decades we in the West have been told to minimise fats and proteins which are non-inflammatory, Whereas carbohydrates like potatoes and carrots and beets and bread and pastas are inflammatory; and should be consumed with an awareness that they are inflammatory, and are implicated in a wide range of modern diseases. The French paradox so-called is a function of food being pretty much a religion in France; with the consequence that the French listen to all the "advice" they are offered, and politely ignore it. They always have. They continue to eat their cheeses; creams etc' in moderation which are filling; and which of course; displace in great measure both carbohydrates; and the ups and downs of blood glucose levels; and even more importantly the tyranny of glycation. I don't know how long it takes for body cells to recover from glycation sufficiently severe to induce a diabetic or pre-diabetic state. but I have suggested four or five months and have not been contradicted. It would be an interesting study. Assuming the patient is on a very very low carbohydrate allowance of string beans broccoli sprouts, that sort of stuff plus of course fats and proteins. For those who are pre-diabetic or formally diabetic, the prospect of amputations and or blindness at some point in the future must be very frightening, that glycation and all the other inflammatory symptoms simply stop if there is minimal or no more glucose entering the body is a blinding flash of the obvious too often ignored.. Perhaps explore the glycemic diet if time and inclination allow? You're not the first; there is a very large tribe of people out there who are in a pre-diabetic or diabetic state.. I'm 75 and I have no sign of diabetes; but I have cause to be cautious, my father died at 67 diabetes complications, so it's in the family. Occasionally I martyr myself. I have what I call a martyrs lunch, which comprises; A glass of red wine with a decent cheese; (very little sugar in red wine) and if I'm having a career day I might add some sliced tomatoes; I've even used lettuce leaves instead of biscuits. There are workarounds., Of overweight and all those bellies out there, just remember that grizzly bears get their pre-hibernating fat from carbohydrates; not fat or protein, there isn't enough of it about. Those are two creators here that are pro Keto. Dr. Berry will give you the medical side of all this and keto connect has great recipes

      ​@@joc8092you're probably at least partially right but it's an absolutely heartless thing to say you have no idea how life affects various people and type 2 diabetes can go undiagnosed for a long time in fact he can go undiagnosed because our lack of health Care in this terrible country until somebody is dying or dead. Did you ever that? You know who are often victims of type 2 diabetes and die very often are native Americans and I live in a part of the country where there are native Americans and they don't get health care. Also fat people usually suffer from a thing called poverty they don't have the money to buy good food because you know what is cheap noodles macaroni and cheese white bread . Fresh vegetables and fruit are not cheap. Take a walk and reconsider your humanity

    Watched this 3 times and it still upsets me. Great honest and humble talk. When I am with obese patients in the hospital this video pops into my psych every time and reminds me to remain empathic.

      You would probably be delighted to know that Peter actually found the patient, as far as I recall. I follow him rather closely, he's a wealth of knowledge and is wonderful in his "calm-ish", rigorous approach. I don't see him as a role model, but I see him as a wonderful proof of concept that yes, these values and this way of thinking can work really well in modern day setting, while internet often present another, rather grim picture. I don't want to live in that picture, so I aspire to guys like Dr. Attia.

      I hated the way some of my coworkers treated obese patients when I worked at the hospital. I would take time to go back into the patients’ rooms to comfort them. We have got to stop passing judgment on others when we’re oblivious of their conditions.

      It's really great that a doctor has corrected himself. And it would be really great if we didn't have to celebrate that, but it were simply part of our education. So, great and kudos. And why have we been "debunking" all the naturopaths, who spend just as long studying medicine and HEALTH, for so long??? There isn't a naturopath around who hasn't been talking about insulin resistance (which most doctors won't even test for, they wait for it to become diabetes when they can prescribe meds) for 25 years or more, who hasn't been recommending proper diets, and suggesting that PEOPLE REALLY START TO CARE ABOUT WHERE THEIR FOOD COMES FROM, HOW IT IS GROWN, and so on. Yes, he's a very well-spoken, handsome, intelligent, educated, and seemingly kind person.

    I cried right along with him. What a stellar character this doctor has developed, more so, than most over a lifetime! What an honorable man he is to review and learn from the past. If only more doctors, more men, more women, were more like this doctor! He is golden! ✨⭐️✨

      Absolutely this talk has aged well. From a historical perspective this is important to have watched for the first time in 2023. The food system is completely contaminated and corrupt. Governments will need to step up and do their job to correct this unmitigated health care disaster.

And if you pretend carnivores think you're eating so healthy, just remember, the Inuit who eat a high meat diet in Northern Canada from hunting, all die on average, at the age of 68 and their bodies all show varying levels of heart disease.@@Talletc

    A year later, I am still touched by this talk. I am a nurse and a recovering obese person. This info gave me hope that I could become healthy. I'm down 81 lbs! The keto diet has reversed my insulin resistance and life is amazing. I believe "settled science" ought to scare all of us in healthcare. Realising I was wrong about eat less, move more has opened my eyes to opportunities to keep learning. Thank you Dr Attica.

      The science was settled decades ago. Type 1 diabetes is caused by an autoimmune reaction to cow milk proteins and type 2 by intramyocellular lipids that inhibit the insulin response. But the elimination of most of this unnecessary suffering by simple dietary changes would put big pharma and their pill pushers out of business.

    This is without a doubt one of the most powerful videos that I have ever watched. I greatly appreciate all of what Dr. Attia publishes on the ketogenic diet and related subjects. He is brilliant and an incredible presenter but my one criticism has been that he hasn't always showed a particular kind or sensitive side in some of his interviews. Having watched this I'm now embarrassed at having once thought that.
    I feel incredibly indebted to Dr Attia for sharing this. He gives me hope for future generations.

      A lot of the Keto/Paleo crowd support eating lots of low-starch vegetables as opposed to just eating lots of meat like Atkins did, and I fully support them in that part of their diet.
      It is probably also true that given the three sources of calories - fat, protein, and starch/sugar - if you drop one of those from your diet it's easy to lose weight.
      The unfortunate thing is that they drop starches when they should be de-emphasizing the other two.

      Dr Attia, please consider that the body hoards fat when it has excess toxins to keep away from our organs. It tucks the toxins in the fat cells & is extremely reluctant t9 give them up, as then it has to figure out a way to deal with the toxins, including the pesticides used in corn & grain production & the GMO'S found in same. Reduce toxins such as mercury amalgams, junk in shampoos, cosmetics, cleaners, pesticides, etc. And eliminate GMOs.

      Whatever we believe, SB G's comments were about weight. "My insulin resistance led to my weight gain...not the other way around." Unless you personally know SB G or she directly responds, we do not know whether she was slim or significantly overweight at the start. There are slim diabetics, you know... So until we know her weight, there really isn't anything to judge as far as her lifestyle because we don't know what her lifestyle is.
      Ex: My mom is diabetic and small. She has mainly cooked homemade meals for the majority of my life. She became diabetic around her 60's. She does not sit around all day stuffing her face with junk and she is definitely not lazy as she still does the gardening, all the cooking, most of the cleaning, sleeps upstairs, and runs her own business in her 70's. I'm not saying SB G is like my mother, only that we don't know enough to make any presumptions.
      I, on the other hand, do stuff myself full of junk and sit around most of the day at a desk. I snack to stay awake at a computer lol. I'm also now overweight due to this lifestyle. I'm nearly 40 and not yet diabetic...but several of my family members are and without eating as badly as I do so I am now actively changing my poor habits before I'm in a worse situation than my mom. If you made it this far, thanks for reading.

      Most doctors take less than 7 hours of nutrition classes. They have tried to lobby for more to be required but the push back and big pharma keeps it from happening. It’s very frustrating!

    I APPLAUD YOU WITH MY WHOLE HEART!!!! I have studied nutrition, weight loss, physiology, and exercise for 30 years to try to figure out what to do about my weight. I have tried a multitude of things, all while being rejected and taunted because of my weight. I often wished to just end it all, but gratefully there were a few brave and loving souls who accepted me despite getting up to over 400 pounds before I finally sound some answers that have been working.     What changed my blood sugar levels was the removal of maltodextrin. I was drinking 6-8 bottles of flavored water a day. Eating healthy, watching the "white" stuff in processed foods. My sugar was constantly over 160 every day, some days it was over 200. I couldn't figure it out. Then I started looking at the "chemical" part of my diet. Ever since I stopped drinking the flavored packets in water, my sugar levels have been down to 119 on the high end. My resistance has improved, and I am losing some weight. So he may be onto something here. Great information in a very powerful presentation.

      My sugar levels changed by cutting back severely on sweeteners like Splenda. My weight is back to normal. I am convinced that these artificial additives raise blood sugar. The more diet food and drinks I consumed, the more weight I gained. The food and drinks industry is killing us with these additives.

      @@spinLOL533 I can't tell if this was a joke or trolling or serious, but if you were serious, it seems like all you know about medicine comes from the TV show "House". As a medical student myself, I want you to know that the kind of medicine shown on TV is very very different from real life and in actual practice, it's nearly impossible to be a good doctor without having empathy and good communication skills. Maybe a pathologist or radiologist like Dr. House would be okay, but any doctor who actually interacts with patients like he does on the show would be unable to do their job.

    We begin our road to being wise when we recognize that we don't come close to knowing everything. As a young mother back in the sixties, Adelle Davis became my mentor. And because of her, I never did pay much attention to that "approved" food pyramid. The introduction of that horrible guide was the beginning, albeit slowly, of the obesity epidemic. Those who studied nutrition from the accepted experts ended up doing more harm than good. Ms. Davis went to actual scientific studies (that it seems most professionals either never heard of or ignored,) and wrote several books on eating for health including the dangers of eating refined sugars -- which includes refined carbs which turn to sugars. I bless that woman for opening my eyes. I am still learning; something we should never stop doing, and am thoroughly encouraged to see that there really are still open minded, ready to keep learning, doctors out there. Thank you.

    I wish I had an empathetic and understanding Dr. like you! It's heartwarming to see you pour your heart out to the patient that you felt you judged. Personally, I cam across your video while searching for an cure for alzheimers. My mother passed away this past July with Alzheimers disease at the age of 91. It was a long heartbreaking 3 years watching her wither away. Im only 61 and have been noticing memory problems. I'm hoping for a cure.

      I was told I had fibromyaligia/ME and chronic fatigue. I was also diabetic and arthritic and had IBS, and had a lot of migraines. I dropped the wheat and sugar from my diet and went 'mostly' paleo. Just THREE years ago I was a mess of constant pain and I couldn't work, I was going to get one of those ambulance call for help buttons and needed a rail to help stand up. Now I work 7 days a week (40 -50 hours) at a very physical job, and go out to concerts/shows etc at least once a week. My pain is gone, my fatigue is gone, I'm now considered pre-diabetic no longer actually diabetic (and the specialists and doctors told me I couldn't reverse ANY of my diagnosed conditions!). Drop the carbs from wheat and sugar. Eat a lot of colourful veg to get all the vitamins. Ditch all the 'man made' fake food - the ones with lists of ingredients you can't name!
      Sugar (wheat breaks down to sugar - putting it simply) causes inflammation, inflammation causes pain. Inflammation caused ALL of my previously diagnosed conditions. I went from a handful of pills twice a day that the doctors had prescribed - to none. Good luck!

      The biggest problem is that there is a huge overlap between chronic pain and drug seekers. For every legitimate patient there are probably a couple of drug seekers.

Until it changes you feel invincible and not a person on the planet could even begin to convince you that you've may not have found THE long term resolution. So I won't even try.       I often tell people that I wish I tuned bright purple when I was in pain or something so that there was a visible marker to its intensity. Because pain is invisible, nobody but you knows how much you're suffering. What's really funny about human perception is how much attention, shock and concern I receive after getting some cupping done (looks like I lost a fight with an octopus) which doesn't hurt at all but I'll be at like an 8 pain level almost in tears and am still expected to work, take care of my kids and house and be a happy, positive person. It's tough.

    We all make mistakes but only few realise their mistakes. My eyes had tears when he told the whole thing & also how it can't be undone what he did.
    I salute doctors who keep humanity at the top & how he felt deeply for his patient.We need more of such person in the world♥️

    It takes a lot of self awareness, curiosity, and introspection to be honest with yourself about past mistakes (especially so as a medical professional where people's lives depend on your decisions and their hope can hang on your empathy).

      My best friend is a Diabetic. Not only do medical practitioners treat her in a condescending manner, but her family do as well. I have done everything I can to try and understand this condition and help her manage it. When we have a BBQ or dinners, the whole diet is Diabetic friendly. I do this so she doesn't have to decide what she can and can't eat. When we go out shopping, I will prepare a meal for both of us that is also Diabetic friendly and we sit and eat it in the car. Her Diabetes is her whole world, it rules her life. The only thing I can do for her, is occasionally give her a time out from this dreadful condition and let her eat without stressing over it, or sitting there having to say "no" all the time by offering food that is inappropriate.

      starquant once your diagnosed “diabetic” here in Australia they put you on a government register and consider you a big burden on the government , insurance companies will black list you no one will give life insurance and doctors treat you as a useless hopeless patient

    Wow. Every single doctor should watch this and learn a very valuable lesson. My mother was in the hospital a few years back. She was in pulmonary distress and it took 4 hours to get her to a point where she could breath as well as possible. She has a long history of pulmonary issues. Not just caused by many years of smoking. It was also due to childhood illness and other issues as well. She had quit 30 years ago. When she was able to speak and the doctor asked if she smoked and she told him yes but had quit 30 years ago he hit her on the leg with his arm. A type of punishment. It took everything in me not to jump across her bed and show him what I thought of his reaction to this poor frail woman. I, and so many others, are at the mercy of these doctors playing God. I am sick of this attitude they have. This is such a breath of fresh air and I commend this doctor for his "truth". Bravo!

      I'm very sorry, they blame everything on cigarette smoking ... meanwhile how many people have lung cancer, emphazema, etc - never smoked or near second hand smoke in their life? - Explain that, so called "Science"?

    Very eye opening. I lost my father to complications from diabetes at 15 and although he didn't really look after his health as much as he should have, a lot of the time, there are things going on in the background which aren't as simple as it seems on paper. Socioeconomic factors, familiarity, environmental stressors, mental illness, level of support, family/friends, life, all play a role in our behaviors which can lead to declined health. Some people struggle more than others and it's something I wish I had realised when I was younger before condemning my father as useless before he died. I know work as a health and exercise consultant/coach helping those with chronic health issues, and the biggest tool to people's success is compassion and empathy - being treated as a human seems to motivate them more than being judged or being told they're garbage. Go figure
