I was on the BSN team when Dallas McCarver was there. He died at 26 years old. I see a lot of similarities with Sam, especially with the breathing. Dallas couldn't even hold a conversation while sitting down at the last Arnold we did together without breathing extremely heavy.
I hope Sam is monitoring his health very seriously. To die or have health problems at such a young age is not worth the social media fame.
You are right Captain Bahbell. But when you are young, you feel indestructible: 'it's not going to happen to me'. Saying "Stop steroids/heroin/whiskey" sounds simple but the physical and mental addiction that comes with bodybuilding is very similar to alcoholics and drug addicts.
When your body is all you are, the psychological repercussions and blocking are very hard to get through. Bodybuilders whistle by the graveyard all the time: cross your fingers and hope it doesn't happen to you. And the addiction is so strong that even when a Dallas dies, it quickly gets pushed back from the conscious and rarely forces serious self reflection from bodybuilders.
If you ever saw Greg Kovacs have trouble climbing 5 short stairs, you recognize that laboured breathing that obese people and bodybuilders have.
I'm happy that slightly older & experienced fitnessinfluencers like yourself are showing concern. Personally this feels a lot like amy Winehouse last tour. You can only watch the car crash happen in real time.

You are totally right about the addiction. TBH it's one of the reasons I never wanted to touch gear. Knowing that you will never look your absolute best unless you are running gear is a serious psychological condition that a lot of men face.
A kid I grew up with at my local gym ran gear off and on through teens and 20s. Was big/shredded and looked amazing. Ended up with cancer in his 30s and he beat it. He was told to NEVER touch gear again because the cancer would most likely come back. But the body dysmorphia was just too much for him and he died a year later after jumping back on a few cycles.
If my 16 years of creating fitness content can teach any of you anything, please let it be this. Cherish living a long life, starting a family and having MANY hobbies. Not just fitness / being a social media influencer.
There is so much life out there to live once you put the bahbells down (on a rest day of course 😊).

My workout partner has used gear since high school. He’s 47 and suffering heart failure. I never touched the stuff but it wasn’t easy. Every guy using could out lift me, I was always the smallest guy at the gym but at my age I am the only one who is in good shape now and healthy. I’m so glad I didn’t fold and go on gear.
Good choice, there's no reason to do it unless you're competing at the top level. The downsides are just too great.

I met Sam during breakfast at last year's Arnold Classic in the UK. The young man was well spoken, and yes his size was impressive at such a young age, but the one thing I still remember was his skin. The acne was on another level and literally covered the entirety of his skin. As a medical professional I can tell you that the skin is an organ, quite similar to the internal organs of the gastrointestinal system, and is therefore a reflection of our internal health. I was and still am seriously concerned for this young man's health. He is damaging himself more than he likely knows
There are thousands of young guys having very bad acne without gear, smoking or alcohol. Some people just have bad genetics when it comes to having acne etc. and you as a "medical professional" should know that.

Just a technical about the heart here:
- If your heart gets larger, that's called dilatation.
- If your heart gets tougher, that's called hypertrophy.
The difference is if your heart dilates, the muscle tissue gets thinner. If the heart hypertrophizes, the muscle tissue gets thicker. Both result in impaired ejection fraction, but the mechanism is different.

His breathing sounds labored to me.
My dad had an enlarged heart. Because he was a sports instructor in the army, but never did PED. His heart was still strong and good at 90 years old. When he was 90 the doctors said he had the heart of an 18 year old. Couple of years later the heart valve was leaking because the muscle was to thick or something and couldn't close it fully anymore. But the doctors told him he could easy get 100 years old with it. My dad wanted that on paper with guarantee lol.. Well. He didn't make it to 100, sadly. He passed away 14 months ago at the age of 95 (probably an aneurysm where they did ssurgery on).

@rockyevans1584 thank you. Yeah normally it's quite old, but I never felt that way. I've always had an older dad. I mean he was already 60 when I was 7 years old. He was never old (thank God) and he worked 7 days a week up until his sudden passing. When I was hitting the gym and he was already way in his 80s he still was way stronger than me at certain things (it amazed me, cause I for example could bench twice my bodyweight) he also had way more stamina than me, while he was in his 80s and I was in my 20s😵(work days of 14 hours or more).

It’s funny because I have had worse body acne than same WITHOUT using steroids. It’s probably why I never touched steroids knowing my body acne could get much worse. Body acne doesn’t necessarily mean he’s extremely unhealthy.

@ I agree, but judging by Sams acne scars on his face he probably had severe acne BEFORE steroids. So it’s not like it’s just steroids destroying his health. Obviously it’s bad for you but not his specific cause for skin issues, although it will make it worse.
Yeah some people are so ignorant. Correlation does not equal causation. Just because Sam takes steroids and has acne, that doesn’t mean the steroids inherently caused the acne.
All else being equal, the steroids have *likely* worsened his acne and it’s *possible* the roids caused it, but you can’t prove anything.
And regardless, even if 100% of his acne was caused by the steroids, that still wouldn’t necessarily signal anything about his general health or health of any specific organ, *including his skin health* - acne does not mean your skin is generally unhealthy. Genetics overall play FAR more of a role in developing acne than any one individual environmental factor, which is why plenty of steroid users get zero acne. It’s just that roids increase your risk of having bad acne

Hey, medstudent here. The problem with ventricular hypertrophy is not actually that the heart has to pump harder to distribute the blood throughout the vascular system, rather the muscular walls of the heart get so big, that (simplified) the ammount of blood transported through the coronary arteries is too little to sufficiently supply the heartmuscle with enough oxygen! Hope this helps :)

Greg, here's a secondary POV you may not have considered. As a 44 yo ferocious defender of being natty (but have not always been so), I fully understand why Sam is okay doing it. In life, we're brought down countless times. Let's be real: few of us won't have to fight depression. Few of us will have fullfillied relationship, financial, social, professional life. A lot of us will get to older age alone, depressed, confused, no friends, no soulmate, nothing has gone according to plan, nothing has taste, fewer and fewer things will feel worthy of getting up for another day in the morning.
Sam is probably fully aware his life is getting shorter, but who cares, in the meantime he will get no financial worries ever again, no girl will say no to him, he won't come accross life as invisible (like most of us are, most of us - if not all - are invisible). He will feel his life has a point, unlike most of us getting to older age without any much to fight for.
Some of us will live longer, but will get diabetes, cancer, arthritis, mental depression, obesity, low self esteem, heart attack, alcoholism, etc, any way. From 50 to 80, most of us will be broken anyway.
So, why not enjoy life to the fullest while it still matters? I get him. And believe me, I'm a fierce defender of staying natty. And I still get his point.

I'm a lifelong strongman enthusiast, which one day used to dream of competing (the only part of my life when I blasted for a few years, other than that, been mostly natty). I admire genetics, so I admire strength and size obtained from your genes: that means naturally. I'm quite a smart person, been a photographer, a filmmaker, a loving and friendly person, I used to be romantic (in many shapes and forms), I'm articulate, I expect a lot from life from a mental and physical standpoint. Nothing of that has kept away depression or illness (diabetes, for example).
Nowadays, being an adult at 44 yo, I see life promises you nothing. By the time you get older, you'll mostly conclude your dreams have been denied, you will not get the love of your life, you'll not live an adventurous life, you'll have no exciting things to tell about from your days on this planet. You'll most likely be broken, physically, socially, financially.
Because life made no promises to you. You just believed it because you were once naive. Even if you fight for the life you want for yourself, you won't (mostly) get those wanted rewards.
At least Sam is enjoying life in a way, most of us doing the HARDER way, won't ever be able to enjoy to the same level as he is.
He will probably pay a price at some point, but even most of us doing it naturally, will pay some price anyway down the road. How many 80 yo do you know that are not broken reminders of their former self?
Even if he pays a price, at least life has made him fully happy while it lasted. Accomplishment, happiness... those are promised rewards most of us won't ever really get.

Id rather be 80 and still kickin it with a good attitude than 34 and a Mr Olympia looking body that's actuallt falling apart. And it's very much possible to live into 100s without health issues if you work toward it, just like any othrr physical goal.

You guys fail to realize there are people that actually have acne/backne since childhood that never went away no matter the clean diet and the water intake and even with skin steroids that helps reducing inflammation! I myself always had backne since I was 11 yo and of course with steroids it got worse, I’ve been to countless doctors and they all said some people just have that kind of backne that no matter the antibiotics it will not go away.

Why is it the acne that people say is the reason why he is going to croak? People that are predisposed genetically to acne will get acne regardless of dose.
I got on trt at 26 and regardless of my dose of 140mg week that puts me into the range they deem acceptable range, I still get acne. I get bloodwork every 3 months and everything from liver to kidneys including an EKG for the heart is always in the green. We don’t know what Sams bloodwork looks like. To say acne is a sure fire sign of near death is pretty inaccurate.
True but based on his physique it's pretty safe to assume Sam is on much more gear than 140mg TRT, which has negative implications for overall health. The acne is just a visible manifestation of that.

We had a steroid user (or a "boler" as we call it) give a lecture to my whole school here in norway, when i was like 13. Last of his gym crew he said, the others had died of various things in their 20s and 30s, one of them he told a dramatic story of dying from a heart attack while bench pressing. For me that means that whenever I think of something like tren, i only think of dead men, or unhealthy unhappy out of shape older men. This was before TRT was really a thing as far as i know.
The fact that he was so unhealthy, and showed pictures of himself when he was jacked, sendt the message. at the time of the lecture he was running a dosing website for various PEDs, so that if kids were going to do it anyway, at least they wouldn't oneshot themselves at age 16. This video reminded me of all of this, and if it wasn't for TRT running cover for the aftermath of PEDs, maybe ppl during the social media age like Sam wouldn't be so quick to go that route too

You will lose all your gains. I lift naturally for 23+ years. Even i quit for 5 years. I still look ripped and in great shape. Smaller but still ripped and fit.
Nah not really. I used to juice some in my early 20s and is 32 now and kept all my gains and even added some on only 200mg of test which don’t do anything… for my health atleast. I also worked out 10 years before starting the juice back in the day.
Also my booodwork, blood pressure, rested heart rate was better than most healthy people
@ My Genetic predisposition helps me a lot. I've yet to touch anything anabolic. I havent lifted in a few years. Kept most of my gains. My gains werent supplemented besides protein powder. Im very interested and intrigued in how muscle gains react to the absence of stimulation across the board, including, gear users, supplementers, and all natty, or combined. Thanks for your response.

Also if he started 5 weeks ago then my bet is that at least 10lbs was water weight as well which you drop in the first few days. That brings it to 3.4 pounds a week. Not saying that is a healthy amount I just wanted to point it out. Numbers are usually exaggerated when it comes to these things.
Then you factor in a as well is he measuring at his highest weight before his cut and lowest weight during the cut to pump those numbers?
My weight can easily fluctuate over a range of 5 pounds in a day. It has fluctuated 10lbs between days as well.

I have a perfectly clear face but my back is full of acne and i am not even on steroids. The thought of getting face acne scares me away from steroids alone. Plush i am a smoker and knowing my inability to get off nicotine i will certainly never stop steroids if i ever take em and adding another thing that drains my life on top of smoking wont end up well.....

theres a million reasons for acne but i'm guessing you consume a decent amount of carbs.
@Cptraktorn just enough to give me the energy I need for a particular workout. I'd say low to moderate carbs overall. It's mostly whiteheads on my nose. Doesn't matter my skin routine or hygiene or diet it seems. They just keep popping up. Sun exposure seems to make it flare up but sunscreen also makes it flare up. Dermatologist tried everything too. Been an issue since I was a kid and I'm now 37. Stress may be the leading factor from a chronic pain issue I've had since childhood. All I can think of... working out is about the only thing that helps the pain but the dermatologist said I probably sweat too much lol
I was never that out of breath when I was 3.9% BF and yes I had it actually measured, he’s probably on a ton of Tren which is horrible for your cardiovascular system and not worth the risk and the acne is a sure sign too. He’ll never be able to go natural again, it’s sad how young I see guys getting on PEDs bc it’s easier but it’s very addicting, and could have lifelong consequences. Greg is right on all of this. I don’t get why someone who doesn’t compete abuses PEDs it’s not worth it.
