Rob, on hand 4, did you consider East might have QJ doubleton (or even Q singleton), or do you take that risk and the worst that can happen is you only make 11 tricks rather than 12?
On bd 4 you never mentioned what partners 1 spade should show. To me it says I have a distrbutional hand and don't want to defend
