Ideally you do not want to cut through the hole in the shirt either, because it will be evidence, if possible cut it down the sides and stretch it around the knife, if the handle is small enough.

occlusive dressings come first, then you stabilize the impaled object.

But what do you do if there is an impaled object in the back? Do you remove it or just lay th patient on their side

bob the time traveler lol u r dead wrong buddy. Chances are low but never zero and I have seen videos of people surviving a knife in the neck. Some people can get lucky that’s why I asked what to do in that rare situation

No, never remove the object. Once it goes in there it blocks any vessels that could spurt out bleeding once you remove it
