How can we differentiate psoas hematoma from collection on USG

It can be difficult to differentiate psoas hematomas from other collections on ultrasound alone. On US, hematomas may have a fluid-hematocrit level or contain reticular echoes, but abscess can also have fluid-pus levels that can appear identical. When mature, abscesses may have a hyperemic capsule. Ultimately, clinical history is required. CT or MRI may be helpful, as recent blood will typically be hyperdense or T1 hyperintense, respectively.
