    I have seen some responses that were very judgmental. I have been in dentistry for over 40 years working for a periodontist. Early in my career I had a young man who presented with a similar situation. I could tell he was mortified and embarrassed things had got out of hand by the look in his eye. That look made such an instant impact on me and made me choose to not criticize or chastise him but empathize and just get down to business. I figured he made the effort to make the appointment and do something about it so who was I to judge or make him feel worse. A few days later, in another office I worked in part time 30 miles away, a dozen yellow roses came with a note addressed to me. This patient had tracked me down to bring me flowers and a note thanking me for being the first periodontal hygienist that didn't chastise or make him feel worse. I am still practicing 40 years later and still have that note tucked inside my jewelry box. It reminds me to be kind and empathetic to each and every patient because of the fear, pain, embarrassment they already have prior to sitting in my chair. I try to bring this lesson into other areas of my life where many people are so quick in judging others.

    Yes, these teeth are bad, but I commend him for finally having the courage for going in and getting the work done. I hadn't been to the dentist in over a decade until today, and I was nervous about what the dentist would say about my teeth (and my teeth aren't nearly as bad as these). If this man was anything like me, he probably had lots of anxiety about what the dentist would say or do when he saw his teeth. I can sympathize with this man's plight, and I'm glad he got up the courage to go to the dentist.

    As a dentist of 42 years, I have seen many patients from poor families much worse. It is truly very satisfying to give patients like this a new confidence about their smile. The ultrasonic scaler if properly adjusted for water flow and power is a godsend, not as uncomfortable as you might imagine. It takes a lot of courage for a patient to come in and allow us to give them a new lease on life!

    • @bsh724
      @bsh724 2 ปีที่แล้ว +55

      @@beckynull5226 As I stated earlier, if the ultrasonic is properly adjusted for power and water flow, the water is warm and the vibration from the ultrasonic is no more than required to breakup the tartar buildup. The gum tissue is not traumatized in the debridement of the tartar buildup and we allow the tissues to recover and heal from the tartar irritation before we see the patient again to remove anything that was missed in the initial appointment. If deep scaling is needed, it is sometimes necessary to numb the tissue so that the patient is comfortable during the visit.

It does look super satisfying to fix people's teeth, clean out the junk and getting the teeth to be functional again. Plus it will feel a LOT better for the person after the treatment. It's a win-win if you can stand some of the more grim procedures. This descaling here will benefit this man a lot, he no doubt had toothache which was untreatable with this level of buildup. Tarter is not really an issue for the actual teeth per se! It’s the GUMS and supporting bone structure that are damaged by tarter buildup. Without healthy gums and healthy bone to support the teeth, however, it doesn’t matter how “good they look”. They will still fall out! I get so tired of comments about “how good the teeth look under the tarter”, not realizing that the teeth are not the actual cause for concern when doing these procedures. Yes, you are trying to save the teeth, but that can ONLY be done by dealing with the gum disease and bone loss that has occurred due to the excess tarter buildup. Otherwise you will end up having those “good looking teeth” fall out of your face because they have nothing to keep them anchored in properly.

      Sandra what do you mean by bone loss. Thanks in advance.

    • @sandracheeks1811
      @sandracheeks1811 2 ปีที่แล้ว +12

      @@monicajane7888 sorry, this is going to be long, but I want to give you a good explication: That tartar is calcified plaque bacteria, which also harbors more bacteria, and continues forming layer after layer of built-up bacteria. The presence of that bacteria above and below the gum line triggers an inflammatory response in the supporting structures that hold the teeth in. Those supporting structures are the gums, jaw bone and periodontal ligaments. “Bone loss” is the result of that inflammatory response going unresolved over a period of time (ie. the person hasn’t had a professional dental cleaning in a long time so the tartar remains on the teeth). Bone loss means that the supporting alveolar bone (tooth “socket”) that anchors the tooth in place starts to deteriorate and weaken. In most cases it is a gradual process but will still lead to tooth loss if unaddressed. This is one reason your dentist takes X-rays frequently. The level of bone support around the teeth can be seen on a radiograph. And someone at risk for or with a history of gum disease needs to have these bone levels monitored for changes frequently.
      Once bone is lost, it can’t be regenerated or grown back. Bone grafting is an option in advanced cases but very expensive. That’s why prevention of periodontal disease (gum disease, bone loss) is so important. If you keep the bacteria off your teeth in the first place (brush, floss, cleanings at least every 6 mos), you can prevent this! There are other risk factors as well (tobacco use, family history, systemic diseases like diabetes). Some things we can control and some things we can’t. Control the things you can!
