No need to be worry, just carry your valid passport, return ticket, and fully paid hotel booking till the date of return… visa fees is 2K bhat which you have to pay in cash only. insurance is not mandatory but I advice to make an insurance just for your own safety… Visa form you can download and fill from India also but if you need you will get the form at airport also… Arrival and departure form will be given to you in flight or after landing, so just fill it nicely and do you immigration confidently
