Hiya. Bloke here. Had breast cancer. For every 8000 women that get it, one bloke does. So yeah, lucky me. My cancer fed on estrogen. So I had an estrogen stopping drug. After they cut the cancer out, that is. The idea of the drug was to starve any cancer missed in the surgery. Took it for almost a year. Felt terrible. The drug filled my lungs up with blood clots. So yeah, lucky me again. Taken a year to get back to almost pre-cancer me. So I'm now cancer free. Yeah, lucky me! We are all the same and we are all unique. Cheers.

I remember when I had my first appointment with an endocrinologist, I asked if hormones change your sexuality, and she told me no. In cases where it does change after starting HRT, she said it's probably from feeling more confident and like yourself and your perspective on life starts to change, you become more open and willing to explore and realize other aspects about yourself that you didn't know before because things like dysphoria are so heavy and prominent before it starts getting treated.

iirc testicle size also goes down for cis men who take T aswell cause the testes see there’s more sex hormone in their blood (E, Prog, or T) and so they atrophy cause they go like ‘yeah there’s enough hormone here i don’t need to work’
that’s also why it shrinks for us trans women and it’s why E monotherapy works and also why Prog also blocks T

    • @Nejove
      @Nejove 5 หลายเดือนก่อน +36

It's called the HPG (hypothalamic-pituitary-gonadal) axis. I think it's actually the brain that detects that there's enough hormone present already and stops sending the hormonal signals (in this case, GnRH, FSH, and LH) that tell the testes to make/release more testosterone. If this continues for long enough, eventually the testicles lose function and shrink from disuse, because it's a "use it or lose it" sort of thing.
Some of them work differently, but there are similar "negative feedback systems" for many other hormones, such as the HPA (hypothalamic-pituitary-adrenal) axis for cortisol, which works VERY similarly, and the adrenals can lose function and shrink in the same way.

    • @qwertasdf5044

@@Nejove the estradiol will bind to the estrogen receptors in the hypothalamus to make it secrete less or no GnRH.
I think you mean the HPTA, hypothalamus pituitary testicular axis.

    • @Nejove
      @Nejove 5 หลายเดือนก่อน +1

@@qwertasdf5044 That's how I thought it worked with estradiol and the hypothalamus, but I went general and just said "brain" because I wasn't sure if I had all of the details right. The hypothalamus and pituitary are both in the brain, and less/no GnRH also means less/no FSH and LH, so everything I said was still accurate.
And I've never seen it referred to as the HPTA versus the HPG axis, but maybe there are multiple names for it that are used by different people. HPG axis is a general term that applies to both AMAB and AFAB people, because "gonads" is a general term that can refer to ovaries OR testes.

As a trans woman who's been on HRT for over 3½ years, I can definitely relate. I'm in the U.S. and my doctor never told me about implants for HRT. When I had my consult back in 2020, the good doctor gave me the options of pills, patches, or injections. I've been on the pills since day one. There was a time when I was six months in and I asked my doctor about switching my estradiol from pills to injections, but she felt no need to do that because my E was already in the cis female range at that point. My testosterone has been nuked for a long time now, such that it won't show up in a blood test at all. This whole time, I've noticed breast growth, softer skin, smaller testes, some spider veins around my ankles, brighter eye color, even some period-like symptoms. Obviously, I don't bleed downstairs for five days a month, but the other stuff that comes with periods (cramps, bloating, mood swings, chocolate cravings, etc.) is there. Body hair now grows slower for me. The same couldn't quite be said for my facial hair, but I've also had laser hair removal done. Still waiting for fat redistribution to the right places, though.
My mood is much better overall. I still have bad days, but my baseline is higher than it was when I ran on testosterone. Highs are higher and lows are lower. I cry a lot more than I did before, both for sadness and joy. I was far angrier on T and I always thought that was all guys and mascs ever felt. But I've long since met a variety of other trans+ people, including trans mascs. And it seems to mainly be REPRESSED transfems who experience the anger and depression when running on T. Turns out many cis men and trans mascs actually LIKE how testosterone makes them feel. My precious estradiol makes them feel like shit!
I can't salute the general quite like I used to. I still can, though, just not spontaneously. It has to be induced.
Before I transitioned, I masqueraded as a straight guy. Since starting HRT, I turned out to be a lesbian. I know some girls who've been transitioning longer than me (even dated one of them), and they were surprised that I never displayed any interest in guys at all. By the time I was coming up on two years of HRT, I started dating another trans girl. She's pansexual and was always shocked at how I'd never experienced sexy time before we dated. She was also baffled as to how I've never so much as looked at a guy. She and many other girls I know turned out to be bi or pan after transitioning. One woman I know kept telling me I'd develop interest in men, but that's never gonna happen for me. Maybe it's because she's a Boomer who transitioned under the old Harry Benjamin rules. Prior to the WPATH reformation in 2011, there was this diagnostic scale that Dr. Harry Benjamin (a German endocrinologist and early advocate for trans people) created. It was quite limiting, though. There were six tiers that Dr. Benjamin placed his patients on based on their level of dysphoria and misery. If you didn't land on Tier 5 or 6, known as "true transsexual" (that was the language used back then), you were usually considered a fake or crossdresser and denied treatment. To land on those higher tiers, you had to have known you were trans from childhood, have intense physical dysphoria, demand medical transition, and have lived for at least a year as your true gender without hormone therapy. You also had to be attracted to members of your assigned sex. Dr. Benjamin thought that transition had to make you straight, not gay, bi, or pan. If I'd transitioned before 2011, I'd have had to lie about my orientation and have some guy pose as my boyfriend. I would have risked losing my estrogen for being a lesbian or not dressing "feminine enough". Trans men would risk losing their testosterone if they didn't seem "manly enough". Thankfully, those old rules were scrapped when the WPATH released a whole new standards of care in 2011 (this time, with input from actual trans people). They switched to focusing on specific individual symptoms and divorced gender identity from sexual orientation altogether.

I'm also madly in love with a guy, we're hanging out tomorrow, but i think he's clueless about how i feel 😅

I'm just a regular guy but I take testosterone because past blood work revealed that I had very very low levels for what it should be at my age. Anyway before starting the test I genuinely thought I might be asexual/aromantic like I just felt like I had no desire at all towards the opposite sex and I didn't have any desire either towards men either, idk it was such a strange feeling to have no feeling lol. One of the things I noticed a few months after being on T is my attraction to the opposite sex came back and I actually first the first time in many years felt a desire to be with someone. So yeah I realized how important testosterone is for a guy for feeling those attraction feelings to people, but this is just what I noticed since being on T, all in all life is better now too.

After a bit over 3 years, I noticed my dark brown/black eyes turned a noticeably lighter dark brown after hrt, my eyes are absolutely more almond shaped and my face looks completely different. I don't think I could overstate the changes, they've been immeasurable and far beyond anything I expected after hearing other people talk about HRT. My body is completely different, I jiggle *everywhere*, I actually have a female-typical waist to hip ratio now and way bigger boobs than i ever expected as well. Like my hips go crazy dumb. My hair also used to be so much curlier, now its a lot straighter, silkier, still curly but much more manageable, and my super thick eyebrows are much thinner as well. Orgasms are exactly as described, much longer, more pleasurable, full body experiences, and I can have 2 or 3 in a row, especially when the progesterone is hitting like a truck. After hearing so many people downplaying the effects of HRT and me being scared of the fearmongering, it has given me so many drastic changes that I consider it a modern miracle. An effect i almost never see mentioned is the skin changes - not softening (which started almost immediately for me) but the change in skin tone/colour. I thought I knew every possible effect of HRT but this one caught me off-guard. I was always warm toned, neutral undertone. About 3 months into HRT I discovered my foundations suddenly didn't match anymore, my skin had a new, yellowish-hue to it, and my veins looked green through my forearm instead of blue. Suddenly my skin had taken on a more olive-tone, matching my mum + sister instead of my dad. I love this change, just as i love every other change so far, but it surprised me a lot! I looked it up and found that yeah, that does happen - though I think most trans women will get fairer, but not necessarily have their tone/undertone change

Gentle fascia/lymphatic massage, light aerobic exercise and compression can help a lot with both spider and thrombose veins. People tend to see massage as something just for muscles, but gently massaging the surface area of your body can help a lot with drainage and circulation as well as promote collagen production for more spongy fascia (which promotes better blood flow) and better skin elasticity.

One of the weirder effects that I've experienced was the introduction of a "menstrual cycle" of sorts.
Now, uh, technically speaking. It's not menstruating because there's, y'know, the lack of a uterus, and thus a lack of a uterine lining to shed. So there's no actual menstruation happening.
BUT! The body will process the estrogen and prep you for shedding the lining (because the body doesn't know what it does and does not have, other than what hormones tell it), causing pre-menstrual syndrome (PMS) including headaches, nausea, soreness in the breast, and abdominal cramps.
It was the first major effect that I got hit with after starting estrogen and spiro, and that was a weird experience. I thought I was sick, until I went to the doctor and they told me that it'd be a monthly thing.

That testicle shrinkage explanation left a lot to be desired. Testicles shrink on estrogen for the same reason they shrink in anabolic steroid abusers: if your hypothalamus is satisfied with the amount of hormones in your body, it tells your pituitary to stop asking your gonads for more hormones. When testicles aren't being asked for hormones, they have nothing to do and eventually atrophy from disuse.
If you were to use bicalutamide instead of cyproterone acetate, it stops testosterone from working, so your hypothalamus asks your pituitary for more, and you generally don't see shrinkage

"HRT doesn't change your sexuality" is misleading. It *does* change your sexuality, but not substantially the part that is gender & sex-specific attraction. E and T sexuality and attraction work and feel notably differently from one another. Its more accurate to say that, someone who already has the innate capacity to be attracted to say, men, but their T sexuality doesn't trigger that, while their E sexuality does trigger it because of how it'll make you attracted from different sources of inputs than what T sexuality does. It makes you see things differently, essentially.

I know a few friends that get the Estrogen implants. Making the body simulate the function of ovaries, pumping a lot of estrogen. BUT from what I've heard it puts estrogen at a high count and you're supposed to burn that number down slowly through the year (depending on the strength of the implant). Blood tests are necessary after several months to see how far you are from burning that number down and if it is around a certain number you'll have to go in for another implant, otherwise you might have until the next blood test appointment before the implant happens...this might vary from person to person and whichever place does the thing, perhaps...I'm not sure. Also, said friend is trying hard to see her partner in the UK during June....really hope it goes well.

The problem is that it also doesn't make money here, therefore the pellets are not certified and they are sold by a very few compounding pharmacies upon the gender care GP prescriptions. You have the pellets mailed to you from hundreds of miles away. But it's still a convenient way of estrogen delivery. No injections here either.

Do talk to your Dr about the POTs, but, I wouldn't let it turn you away from E unless your Dr thinks it's a major risk

I forgot to add, the only reason Spiro is common is because it's cheaper.

@@SuspiciousTemmie True, but arguably the best one (cyproterone) isn't available in the US. And there's no real way around things that are effects of E itself. Either you find a way to tolerate them or you can't medically transition.

Oh, the decreased Alcohol tolerance hit me hard. I used to be able to outdrink sailors, marines, and army personnel. But now?!?! Now i have half acan of cider, and it like i had 3 bottles of Jameson.
So yeah, i have basically stopped drinking almost completely. Its so much funnier being the sober friend at the bar.

I never liked alcohol as a drug that much, I only drink for the taste of good alcoholic drinks or in good company.

I've never really had any alcohol (apart from the time when I was young and it was Christmas time) both mum and dad at the time forced both me and my sibling to have some *diluted* but I was getting hooked on it because I remember asking for more, they only went so far because obviously being so young you're not supposed to drink alcohol. They were hoping it'd stop us until we're at least 18, can confirm as I'm about to be 3 years older than 18 and still haven't if anything I don't really ever feel like having alcohol so I guess that's a win in some sense although I know their plan was to just stop us until we were 18
At least I don't have to worry that much about that change based on those circumstances but who's to know maybe I will end up drinking alcohol at somepoint

As someone whos trans and bisexual there is, especially in our society, a difference in whether you date someone as a woman or as a man because we internalized these gender roles so much, that even if you reject those roles you will still subconsciously feel and be seen/treated different and you might for example not fit into the role of a man dating a woman but you might feel much more comfortable being a woman dating a woman etc.
As a personal anecdote: Before I consciously realized I'm a woman I had a "lesbian phase" where I would consume a lot of lesbian romance movies and books (not in a gross sexualizing way) and I thought that was the most beautiful love ever because it was different than a man loving a woman.

In Australia you have to find a pharmacist who will prepare the implants off script but it does end up being slightly cheaper over time
