Scoliosis, a progressive condition, worsens over time, especially during adolescent growth phases or in later life stages. The severity of scoliosis is determined by the Cobb angle, measured on an X-ray from the tilt of the vertebrae, with higher degrees indicating more severe cases. Scoliosis is categorized as mild (10-25 degrees), moderate (25-40 degrees), or severe (40+ degrees), with an additional category for extremely severe cases (80+ degrees). Early diagnosis is crucial, as the progression and treatment efficacy vary greatly among individuals.
Mild scoliosis is often the initial stage of progression, with curves less than 25 degrees. Despite being the optimal time for treatment due to the spine's flexibility and lower severity, traditional approaches often recommend no intervention at this stage, allowing the condition to potentially worsen. Conservative treatments at this stage can significantly reduce curvature and prevent further progression.
Moderate scoliosis, defined by a Cobb angle between 25 and 40 degrees, presents more noticeable postural changes and may lead to discomfort or pain. Conservative treatments aim to reduce curvature using a combination of therapies, including chiropractic care, physical therapy, and corrective bracing. Unfortunately, traditional approaches may still not recommend active treatment, risking further progression to severe scoliosis.
Severe scoliosis is marked by angles of 40 degrees or more, significantly impacting posture and potentially leading to functional impairments. At this stage, traditional treatments often lean towards surgical intervention, such as spinal fusion, to correct the curvature at the cost of spinal function. However, conservative methods continue to focus on reducing and stabilizing the curvature through specialized therapies and exercises, aiming for long-term management without surgery.
The treatment and management of scoliosis require a lifelong commitment, whether through surgical means or conservative therapies. Early detection and treatment are crucial for the best outcomes, emphasizing the importance of monitoring and addressing the condition before it reaches more severe stages. Conservative treatments offer the possibility of avoiding surgery, highlighting the need for awareness and proactive management of scoliosis.

This was an excellent video. I could really use your advice, doctor.
Here is my situation: I am 67 years old and have a 53 degree Cobb Angle therefore I have severe degenerative scoliosis.
I do not want surgery. The spinal doctor who diagnosed my condition is only interested in doing surgery though she did recommend I see alternative treatment.
I have tried acupuncture which has helped some; chiropractor which was okay until he broke two of my ribs adjusting me (yeah, I know) and PT which honestly hasn't done much. Up until the rib fractures i have been doing resistance training my whole life and think that has helped me stave of some of the effects of my scoliosis.
What advice would you have for someone in my situation?
    Many thanks.
    Bill

I am almost identical to you. I found a chiro who specialized in scoliosis. I was even on oxygen because of pressure on my lung. They started me on simple traction, pain free bands that stretched my spine. No more oxygen needed and I became more flexible. Curse reduced by 3 %. Just enough to change my breathing. . For Pain I found the best pain management doc and now get three spinal nerve ablations a year. Fast, simple and PAIN GONE.

My 13yr old daughter was diagnosed with 51 degree severe case. First doc visit (at UCSF) we were told about fusion surgery being the only option considering severity. What is the success rate of this surgery at her age? What are the side affects in her mobility post surgery recovery? Are there other realistic options? Thank you
