Depends.. We had to pay at resort.. However some resorts include travel charges in the package, so verify before finalizing.

Not mandatory..
It covers risks like hospital expenses, if you have to extend your stay and reschedule your flight, any luggage losses in the trip, or cancellation of trip due to any factor.
Considering the amount invested on the trip, it's a protection cover.
