Easiest answer would be that the Doctor himself calls himself the Doctor. As shown in running joke "I'm the Doctor." "Doctor? Doctor who?" "Exactly." I don't think it really matters what you call him, most fans and production team (including at least 10th and 11th Doctors) call him by name the Doctor. I know this answer isn't that good, but if you want to know more you have to check tardis wikia as this whole naming thing has almost 50 years behind it. Cheers!

@twizawia I watch it and I'm near Chicago. I dressed as Amy for Halloween and then got yelled at for not having a costume. My hopes of finding a guy that knew Doctor Who were crushed haha.

@Mika121416 He's a bit young, but then the doctor's always saying he wants to be ginger. So maybe in a few years (i.e. ten or fifteen) when he's a tad older.

It was always 'Doctor Who' until David Tennant insisted on being called 'The Doctor'... so it was him that was 'The Doctor' rather than 'Doctor Who'
