Here is the scary part. Considering the repetition and irrefutable likeness, John must be consciously suppressing anything including Sherlock. What this demonstrates is that John can't distinguish delusion from reality. Sherlocks appearance in his life occurs so frequently and vividly that anything he thinks he sees he immediately denies, no matter how striking the similarity. The real Sherlock could've stood in front of him, and he wouldn't recognize him.
