With only a few minor changes, I have loved every change the series did from the game, except this one. Taking on the raider and having the railing give way that they fight on, with Joel falling 2 stories and being impaled on rebar spells a much more grim picture and sets up what's coming better than being stabbed with a broken and jagged piece of wood.
It is actually incredibly unrealistic for someone to survive the injury shown in the game. Not *impossible* mind you, if you immobilize the person and get treatment immediately there's a chance. But getting up and trying to get away with that kind of injury is almost assuredly death. So with that consideration, I get why they'd change it to something more realistic but similarly dire as how the game portrayed that likely fatal injury.
Given that so many people who aren’t familiar with the games are convinced Joel’s dead at the end of this episode, I think the grim outlook was established perfectly without being too unrealistic. The injury from the game actually took me out of it because of how ridiculous it was that he survived.
