Mistake 1: Didn't lay on the floor and visually check the placement of the jack.
Mistake 2: Didn't take your time. You just kept pumping and pumping without stopping to check things out.
Mistake 3: Didn't position the rear wheels of the jack perpendicular to the car. When jacking up a car the jack needs to roll in towards the car. It was unable to do that as the wheels were facing the wrong way, therefore the car slid off.

I think placement was probably good. What appears to have happened is that the jack was not allowed to roll forward as it was jacked (due to a floor crack or whatever stopping the jack wheel), so instead of the jack rolling forward as it generally does keep in touch/locked with the lift point, the jack saddle moved/slipped back with each pump, until it was too late.

Thanks for posting. Watched it a few times to see what exactly happened so that it doesn’t happen to me. Found the real issues.
I don’t fault you for not looking under the car. They have a small post like jack spot that you can easily feel with your hands as you seat the jack to the post. I can see that you moved the jack around until you felt it centered. Much like a mechanic putting a nut or bolt in a hard to see blind spot (Dexterity). I would have looked anyways, but I can kinda tell you had it seated.
    The problems are the following:
    1. You used two hockey pucks stacked that pretty much slipped as they created a leaning tower. That’s why jacks have teeth or pads with groves.
    2. Car was at a high enough angle to create the slip with slippery pucks stacked. Pucks are meant to slide right?
    3. Seemingly rough and cracked cement that prevented the jack from moving forward which never happened and only moved sideways once the car slipped.
    4. Don’t know if wheels were chucked to help prevent front movement , cars are designed to move forward and reverse, they roll. With that said unibody cars are rigid which is why when you lifted it the back wheel came off the ramp. Long story short, might have been better to lift one side then the other instead of back then front with such a difference in height.
The mechanics of the jack allow it to move forward and reverse easier then sideways. Watch the arm movements. So lifting one side first then the other would have helped with your particular scenario (rolling resistance). If you wanted to lift front or back first instead of the side, then place the floor jack in front or back. Jack will move easier to center itself. There are places on the car to lift from the center front or back.
Hope this helps you or anyone else. Like I said, thanks for posting. I may have made a similar mistake, you just made it first and I decided to learn from it.

Sorry to see this but I’m glad you weren’t hurt. I had the very same thing happen to me with my Camaro Zl1 1LE. I crushed my passenger side fender and ended up having to replace it. Getting the door fixed is no big deal. Looks like you already fixed it. This is a lesson in physics as well as attention to detail. Your jack never rolled under the car as the car was being lifted (same thing that happened to me). So it’s jacking the car at an angle, the higher the car goes the more severe the angle. The second you take your hands off the jack it will roll out from under the car.
Get yourself a quick jack and Race Ramps. Those rhino ramps you are using are known to buckle. The next day after my debacle I went out and invested on the Quick Jack and Race Ramps. The best $2,000 investment you’ll make on your car if you are going to do your own work or film videos.

@@BulletsGarage I've looked in to this. Secondary catalytic converters are part of the emissions control system on a vehicle. They were designed in by the manufacturer to ensure compliance with environmental laws. They are not there for "cosmetics" or "sound deadening". These "secondary" cats still have catalyzing elements like platinum and rhodium n them that make them expensive to add to vehicles. You can bet if they were not needed to meet emissions regulations that the manufacturers would not add them in the first place.
The reality is -- that in some locations - removing ANY emission control advice is against the law -- but getting caught not having it is not "illegal". In some cases you will not be able to register your car. Jurisdictions with progressive environmental laws -- such as CARB states, Scandinavia, Germany, etc. will definitely not allow you to register your vehicle if any factory-installed emissions device has been removed even if it does not cause an illuminated MIL.

Floor jack at the front center jack point (there's a crossmember spot between the wheels). Then put the jack stands at the jack points after. If car is too low, get low (about 1.5-2 inch thick) and decently long (3-4+ feet is good) wooden planks behind wheels and back the car up slowly onto them before using the jack. Make sure you have enough room for the jack points when you do the planks. Can do the planks behind the front wheels for about a foot first when backing before you put in the rear planks for rear wheels. Good luck!

Use a cross member when using a floor jack and consider using a 2x4 or bigger between jack and car to spread the pressure.

We learn from others mistakes, if we are lucky. What really happened here is simple:
    1) Jackpoint was engaged correctly and a puck was used.
    2) Floor jack had its wheels perpendicular to car. < a mistake.
    3) Floor jack raises BUT it is not rolling towards the car.
    4) Car slips off puck or puck slips off floor jack.
What went wrong? Friction force needed to move/roll floor jack forward was greater than the friction force need to move puck off of floor jack or jack point.

I watched the video several time. I think the cracking noise was not from the car but from the ramp. Note that the ramp moves a bit coincident with the noise. Also note that the pads slip towards the front. I think it's because the car was on too much of a difference in an incline to start with and that he'd used two pucks. As the jack moves up more load was transferred to the front driver wheel causing it to sag and increasing the angle as well. The jack wants to stay flat relative to the ground leaving the pucks to adapt to the angle so the two pucks formed their own angle creating an off center load and slipped. The rear casters being at that angle doesn't help either. As the jack moves up in an arch, the caster should roll forward into the car but the casters at the back prevented this. When he moved the lever up getting ready to stop he might have gave it a little twist that caused the tipping point; puck slipping. Lesson learn....don't use two pucks and lift a small height at a time.
