Is there anyway of doing this in a table? When I hit shift tab it takes me to another table on a different page... not sure what I'm doing wrong.

You can set it up in InDesign and finish the forms in Acrobat.

Depending on which method you're using, you could add one of the smaller whitespaces (like a thinspace or hairspace). -EG
