You should check with your bank's website how much "extra" it will cost to get money from an ATM abroad. The 220 Baht is the base price. Your bank can add extra costs to it.
I've exchanged money in Phuket and Bangkok and from my experience the rates are 99% the same in different exchanges, the only thing I noticed was that the rate for larger bills ($100 bills) the rate is better than for say $20 and $10 notes or less. For example for a $100 bill they'll give you a rate of 36 Baht per dollar, and for a $20 bill they'll give 35.5 baht per dollar (just an example)

True - the 220 Baht is the minimum ATM fee you'll pay in Thailand. Other charges depends on your bank.
