Avascular Necrosis, Blood Supply Femoral Head- Everything You Need To Know - Dr. Nabil Ebraheim

  • Educational video describing the conditions affecting the blood supply of the femoral head.
    Where is the important blood supply of the femoral neck located?
    On the postero-superior aspect of the femoral neck. The main blood supply for the weight-bearing dome of the femoral head is the medial femoral circumflex artery (lateral epiphyseal artery).
    Using an awl at the piriformis fossa in children requiring intramedullary fixation of their femur fracture may endanger the blood supply and cause avascular necrosis of the femoral neck.
    Avascular necrosis is death of a segment of bone.
    High energy neck fracture
    •Vertical fracture of the femoral neck that has vertical obliquity of 70 or more degrees. Has a high rate of nonunion and avascular necrosis.
    •Displacement of the femoral neck fracture disrupts the blood supply (kinks the retinacualr vessels). Pressure from the intracapsular hematoma may affect the blood supply.
    Clinical picture
    •Hip is in flexion and external rotation
    •Surgical emergency in young adults to unkink the vessels.
    Treatment
    •Obtain anatomic reduction in young adults.
    •Open reduction if closed reduction is not possible, use Watson Jones approach between the gluteus medius and the tensor fascia latae.
    •Follow the patient with an MRI to evaluate AVN.
    •If the patient develops nonunion and the head is viable, treat the nonunion by valgus intertrochanteric osteotomy.
    Hip dislocations
    Posterior dislocation is common. Anterior dislocation is rare.
    Treatment
    Emergent closed reduction within 6 hours.
    Assess hip stability post-reduction.
    Other factors associated with hip osteonecrosis
    •Corticosteroid use
    •Alcohol abuse
    •Hemoglobinopathy
    •Coagulopathy
    •Gaucher disease
    •Pregnancy (rare).
    Idiopathic AVN
    Nontraumatic AVN of the femoral head is located anterolateral.

    Thank you... I just fell off the ladder and i have dislocated my hip. This is like a horor story. I have 0 money and I may turn into disabled 35 yo. And i worked hard all my life just to survive and pay debts...

    I had this, that followed by a complete left hip replacement!! total success!! I went from 25 hours a day pain to going back to work full time position n less then 2 months!!!! thank you Drs!!!!

      Did the problem show up on xray or other imaging. I have terrible hip pain. Nothing on xray having mri next week. So debilitating. I'm 71 male.

      They were X-raying the wrong area . at first i thought it was sciatica!! so they xrayed th l5 vertebra not the hip. Once I mentioned how bog my hip was getting... A year later.. the did a cat scan on my hip and saw that it was "arterial necrosis" the veins & arteries were all destroyed & the hip was a lil nub.. 😞😞😞
      Thank god for the hip replacement!! I had it done in O8.. It's getting a lil weaker now. but I have very lil pain.. Limp only when it super cold or I'm really tired.

    Three years ago I began to have pain in my left lower spine. Mayo Clinic in Jacksonville described my problem as entrochanteric bursitus and fact problem. I was given a shot in the trochanteric bursa. Everything felt normal for months. Pain did return. Many times since I have received shots for pain; two to three times a year. But the locations of the shots changed. The pain doctor in Charleston injected in the hip area. Now two months ago I am told I need a new hip due to AN. This is the first time I have heard this diagnosis. Hip replacement ssheduled for May 6 at Duke Regional Hospital. My history of left side problems began immediately following an ERCP in 2004. Everytime I ate my left lower back and hip area would seize up. This problem began immediatelly after the ERCP. Standing and walking as I left the table from the meal would be difficult. I saw doctors many times about this, but there were no findings. The area above my waist at bottom of ribs over to spine is affected. I have no arthuritus anywhere else in my body. Thank you.

      Hi Judy I had also the same situation with yours.Last two years I was diagnosed with osteonecrosis until I was unable to climb up stairs and its so painful the left and right hip bone. I try many treatment just to recover but I'm hopeless until I found a product that helps me a lot and for only 1 month of taking there's improvement.     I am having open decompression surgery next friday. I am in stage II of AVN. The surgeon informed me it will probably be a short term fix. Most everything I have read confirms that. Instead of using my BBMM, he is going to use bone grafting material to fill the void. Any Ideas???
