Triphasic MRI abdomen ( Liver abscess in segment VII )

  • Pre and post contrast MRI of the abdomen and pelvis with dynamic study of the liver revealed:
    Provided history of hepatic abscess, comparing to last CT study dated 10/5/2023, the current study shows:
    • Enlarged liver with smooth margin. There is no significant change as regard the previously described large ill-defined focal lesion in segment VII showing irregular thick wall that shows progressive enhancement toward the delayed phase with heterogeneous non enhancing central areas of cystic changes, on the DWI, it shows peripheral diffusion restriction with central free diffusion, the lesion is surrounded by an area of edema and hyperemia. It measures about 5.8 x 6.4 cm. Few cysts are seen at the peripheral of this lesion, the largest measure 1.7 x 2.7.
    • Few other small lesions approximately 3 are seen in segment V and IV demonstrating intermediate T2, high signal on DWI, the largest measures 13 mm and is seen in segment V showing diffusion restriction and post contrast enhancement. The others are too small to be accurately characterized.
    • There is a subcapsular cystic lesion near the falciform ligament measuring 13 mm in diameter.
    • There is no change as regards the multiple LNs seen in the upper abdomen, portahepatis and retrocaval region measuring up to 28 X 22mm.
    • Few left renal simple cortical cysts, Otherwise, The pancreas, suprarenal glands and both kidneys are unremarkable. Mildly enlarged spleen measuring 13.5 cm.
    • Normal caliber patent Portal Vein, splenic and superior mesenteric veins.
    • Normal course and caliber patent aorta, hepatic veins and I.V.C.
    • Stationary course of the multiple enlarged celiac, porto-caval, retrocaval, porta hepatis and para-aortic lymph nodes, largest 22x18mm, some of them show cystic necrosis.
    • No ascites. Minimal right pleural effusion.
    • Normal filling of the urinary bladder with smooth inner surface.
    • No pelvic masses or collections.
    • Few vertebral body hemangioma are identified.
    CONCLUSION:
    • No significant change as regard the size of the known hepatic focal lesion in segment VII, biopsy is recommended to assess possibility of necrotic neoplasm.
    • There are multiple upper abdominal and porta hepatic suspicious lymphadenopathy
    • Few other small hepatic focal lesion in segment IV and V.
