My guess is that he doesn't see what he's doing as wrong cause he's an incredibly selfish person. Frankie's distress doesn't matter to him because it's about what he wants and what will make him feel good. He thinks that the fact he "cares" for his sister makes up for what he puts her through. However, I think he does have enough sense to know he'd get in trouble if the truth were to come out.
By that logic why doesnt she just tell someone? Because He'd make her pay for it ten fold spoon as he gets the chance and he's manipulating as well as abusing her to the point she feels she can't stop him.

He definitely knows it’s wrong maybe not inertly. but she visibly cries, says no and shows extreme signs of discomfort which he’s able to see
