you have a bad battery my friend we get this problem in the store often and the issue most of the time is with the bad battery. replace your battery and you will have no issue. but if you always have access to power, you can use the laptop with power only and there is nothing wrong with it.

do you have a multimeter or ampmeter ?, because its a very rare case that a charger gets damages and cant provide enough AMP,, because the voltage cant drop!, but its very very rare, and i 90% dont think its your charger.
the things that could go wrong.:
#1. common isue battery starts going bad
#2: 2nd common would be a bad windows update driver for A/C. i would recommend you to unistall the A/C charging Drivers. by following this steps:
The power behavior is controlled by your hardware settings.
Issue can also be with the faulty adapter or the charger component on the mother board is not working.
You may try these steps and see if it helps you.
      1. Click start
      2. Type devmgmt.msc in the search box and hit enter
      3. Expand the Batteries category in the device manager
      4. Right click the Microsoft ACPI Compliant Control Method Battery listing and select uninstall
WARNING: Do not remove the Microsoft AC Adapter driver or any other ACPI compliant driver.
      5. Click scan for hardware changes
Windows will scan your computer for hardware that doesn't have drivers installed, and will install the drivers needed to manage your battery's power. The notebook should now indicate that the battery is charging."
Try these and let me know!
if the above didn't resolve your issue then you may have a bad power regulator chip that is on motherboard that is not working properly.
