I have a question, is it good to start with French lesson first or one should do DELF course school for the nurses ??? Since Red Cross asked to show TELC & DeLF B2 ...Do the French teacher explains in English?

Bonjour. Take the French lessons first. Then do the exam with DELF. A teacher who does not use English to teach you French is the best. You will learn faster.
