Your setup is pretty good. Great posture. Not everyone has this. Maybe standing a touch too far from the ball. Right before you hit it you really reach for it ... can cause you to pull inside on the downswing. This outside/inside produces your fade -- or a pull hook if the clubface is shut. Try standing a little closer at address. Club back and through on plane.
Need to take you left shoe off since you keep missing the read to the left all day!! LOL!! It would have allowed for a better read!! :P Still play better than this guy btw!!
