The more the viewpoint of Helga regarding Arnold goes on, it almost seems more like it’s her show rather than his. And yet, he’s supposed to be less the character with the story, and more our eyes into the world of all these weird, eclectic people.

Her name is Russian I think. It's the same name as her sister Olga (Olga is the German version of the name "Helga"). I may have the countries of origin backwards though.
Why can't Helga Pataki ever tell Arnold that all he ever cared about is himself in any of the Hey, Arnold! episodes? How come she never said it to him? That's what the world wanted to know.
Arnold is not that self centered, but if you mean why she never told him she cares, then that's because of the reputation she has built around herself as a protection against anyone who tries to bully her. It's actually sad if you think about it.
If you mean why she never admitted her feelings, in the one episode Helga on the Couch, Helga admits to being very insecure despite her "tough girl" cover. She may act tough, but she's very insecure and thinks he'll reject her; given that he's basically the one good thing in her life, that would crush her. Hence, she's scared and would rather live with always wondering as opposed to facing rejection and being emotionally destroyed. It's really sad and a rather mature thing to put into a kid's show.
