    That knee was swollen 3x its normal size during the Knicks 76ers playoff series.. and he played on it every game the entire series, then continued to play on it a month later for the olympics.
    Front office should all be fired for allowing him to play

      fans would have called him softer than they call him now... he makes millions so hes supposed to play until he dies in their eyes, only thing that matters is basketball and the players real life is irrelevant... i still got screenshots of people saying kobe ruined the lakers because "he tore his achilles" im convinced nba fans are some of the worst humans on the planet.

      If my back hurts, I have to go to work. If I have a flu, I have to work. If I have fucking cancer, I STILL have to work. Get a grip. If he cant play, just retire (actually a possibility for someone making 50 million a year). Instead, he'll drag this out as long as he can while fucking over the 76ers so he can get more money.

      @teledude1owns "Allowing him to play!" As if he's not responsible for what's best for him as well. The Sixers front office isn't his mom! They can make suggestions but, they can't tell him he cannot play in the Olympics. Also, as Dr. Sutterer pointed out, he's going to hv windows where he feels fine and, no amount of rest is going to cause his meniscus tissue to grow back!

      @productions5667 Embid is, and has been, lazy and indolent in everything he does, from the slow and laborious way he talks and processes information, to his slothful demeanor and the way he jogs back on defense all-the-time. Also, Embid's work-ethic criticisms predate his injury issues. He's always been known to do just what was required, nothing more, and usually much less.
      Finally, there are ways of maintaining your conditioning without engaging in high-impact activities. Swimming is one! There are also specially designed treadmills now that athletes can use that allow for players to reduce impact on knees, hips, ankles, etc. that allow them to maintain cardio while undergoing lower extremity physical therapy! You are either ignorant of the above or, you're just making excuses for Embid!

      You need to take out the meniscus if it is damaged you can't leave it in there. What they should have done was to try to replace the meniscus by using cadaver or stem cells because I'm pretty sure he has a bone on bone situation

    Great channel. My husband is an ER doc and he cannot deal with most med sports commentators, but he really likes your channel. He appreciates how you truly educate fans…so many others only offer hot takes and clickbait. I am wellness/fitness director who has a labral tear in my hip(wish I never had surgery didn’t work and ended up worse off), shoulder separations, and have to wear a knee brace anytime I teach classes so I get the whole range of injury management vs. surgery.

    I'm 7 weeks post-op from a meniscus repair and can certainly sympathize with Embiid. I don't fault Embiid's surgeon at all for the meniscectomy because, as my surgeon put it, "you can't sew together sawdust". I was fortunate enough that my surgeon was able to stitch together my tear in the posterior horn of the medial meniscus, but recovery from these surgeries takes awhile.
    The nightmare scenario for meniscus injuries is basically what happened to Lonzo Ball, or Carey Price in my sport of choice (ice hockey). In Price's case, he went in for a meniscus repair/meniscectomy just like Embiid or myself. However, during that surgery, the surgeon discovered an osteochondral defect; that is, the cartilage had worn completely off the femoral condyle to the point where the femur could no longer glide appropriately or smoothly on the meniscus.
    A potential video idea for Dr. Sutterer is talking about a surgery that used to be done on athletes for these sorts of injuries (microfracture knee surgery) that is no longer performed for a variety of reasons. Guys like Tracy McGrady and Greg Oden say the surgeries prematurely ended their primes. Nowadays you see procedures like OATS and MACI, such as in the case of Lonzo Ball.

      @@cecagna micro fracture is still performed, it's what I had done last year for delaminated cartilage on the tibial plateau. It helped but it still hurts. This was at the same clinic that treats the blazers, so maybe Portland surgeons are behind the times.

    My dad played basketball his whole life until college (back then in Converse Chucks, lol), and he has had meniscus issues and multiple surgeries, too. Any time an NBA player has meniscus issues we grimace, because we know that's a tough one to bounce back from. It really, really sucks.

    I had a similar surgery, a right lateral meniscectomy. From what the doctor at the time told me, a meniscectomy is often performed based on where the tear occurs. Parts of your meniscus don't get very good blood flow because it's pinched (the parts that are closer to the center of the knee), so even if doctors go in to repair it, it will never heal because it doesn't get enough blood flow.

      I wouldnt say never… just takes a long time. I had a lateral repair and it took 5-6 months just to get somewhat comfortable. And a year and half before i felt normal.

    Regardless of sport code, unless the knee's strength is proportational to body weight by nature, big guy's knee tends to expire earlier than normal. The loading shouldn't lie.

    I had meniscus removal 16 years ago. Knee swelling happens so fast from rigorous activity no matter how good your knee feels.

      @ it’s fine doing day to day stuff, but anything I try to run or do anything more rigorous than the usual i can feel the pain and swelling soon follows. I’m sure it’s different for everyone but I remember my dr telling me “don’t try to be a runner” and I shortly understood why.

      As much as I have hated all of Embiid's bad behavior and controversies, unfortunately his nba career might be cooked, hopefully he will be able to live a normal life after whenever he decides to retire from the NBA, because as a lot of other people have commented here it's a painful process to go through. I feel for Embiid and his family, they are the ones that usually are the more affected😢

    I genuinely feel sad for JE. Some dudes their body just ain’t built for long term physical contact sports. It doesn’t make them weak or less than or bad they’re just not a good fit for that type of job. Much like someone with a low lung capacity wouldn’t be a good swimmer or someone who hates blood shouldn’t be a doctor. Certain people just biologically anatomically are not meant to go into certain fields and that’s ok. They can be excellent at other things and still have meaningful contributions to society just maybe not how they envisioned.

      It's definitely unfortunate but like you said, his body was not built to play basketball for decades. Now, his knee(s) pain will only get worse as he ages. This will go on way after basketball.

    There were concerns with Embiid's health before the draft. He missed his first two seasons. In his rookie year he only played limited games which is the only reason he didn't win ROTY. It's a surprise to me that he became the player he become, an MVP and perennial allstar with playoff runs.

      @@leonardoyi3183 We know Jokic should have another MVP but we all know the NBA don't like to give it to the same player too many times. Embiid was at least one of the best players in the NBA that season and that's my point, nobody saw him becoming a top 5 player in the NBA.

      I feel this year a little more fans were frustrated with him, myself for one. The Olympics, followed by the extension, followed by missing the start of the season, the back-to-back comment, now the knee injury is back. This is a PR disaster by Embiid and the front office.
      The majority of hate is from the rest of the league that are still bitter about being an MVP.

    More and more times I am hearing friends of mine regretting having meniscus surgeries. I teared up my meniscus on April 2023, ACL, PCL also teared up (even tho I believe this was already there, more about my own wear and tear, I have been playing sports for the last 40 years). After one exhaustive month of research and study I decided to don't have surgery. The reasons were: my age at that point (46), the amount of bad experiences close people had post surgery and the not high percentage of success in middle and long term patients post surgery.
    Most people get another surgery after the first and a few recover completely (if that's possible). I took a conservative approach; one full year of no impact sports, self rehab and tons of mind training. Nowadays I play basketball and football/soccer again. Yes, my meniscus "clicks" at some positions and when i do certain deep movements in motion, however I can still play, I don't feel pain or unbalance, my knee doesn't swell at all. I have stopped other work outs like animal flow or techniques that requires me to be squatting for long periods of time but I am extremely happy that I didn't succumb to my doctors opinions. All of them told me to go through surgery, mainly because that's how they make their money.

      While Embid can't necessarily be blamed for "being injured," he definitely deserves abundant criticism for never having the pride and discipline to maintain his conditioning over-the-years. That alone could have helped create the conditions for him physically to sustain these injuries, or at least exacerbate them once they occurred!

I know you would have to give Joel the benefit of the doubt in that scenario and lord knows you probably don’t want to do that but you think that could be a possibility?

      @Mugga94 Joel is lazy and indolent in everything he does, from the way he slowly talks and processes information, his constant slothful demeanor, to the the countless times he just jogs back on defense. Also, issues with his work-ethic predate his injury issues.
      He's always been known to do just what was required of him or, much, much less. Lastly, I'm sure there were times he was behind the eight-ball in staying in shape however, there are multiple means, like swimming, to stay in great shape. One doesn't always have to be running and/or weight-training.
      Regardless, Embid is hardly someone who's earned and deserves the benefit-of-doubt over-the-years and, he definitely doesn't deserve the contract he's playing under but, that's more the organizations fault than his. Thanks for responding!

    I didn’t know his knee was that bad. It reminds me of Jani Hakanpaa’s knee injury in the NHL.
    Toronto and him agreed on a contract in July, but it wasn’t officially signed until a week or so before training camp because the team wasn’t sure he’d be able to play at all. They described it as he didn’t have a meniscus in his knee so it was basically bone on bone. And that it wasn’t anything that could necessarily get worse but it was just a pain management thing. And so far he’s only been able to play in 2 of Toronto’s 21 games so far

    repair vs resection is often a *choice* as long as you're prepared to accept a long recovery. seems to me the front office is happy to gamble on getting one good season at the expense of it maybe being the last. i'd be much happier if they rolled the dice on repair, and they get what they get after however long it takes, and everyone lives with it. i think the league could probably change the salary/contract structure to incentivize keeping a player on ice. someone as big as embiid has inherently fragile menisci, relatively speaking, but people seem to expect elite athletes to heal like an 18-YO with an ankle sprain

    The loss of meniscal tissue increases joint loading and accelerates degeneration, especially in the lateral compartment. I'd love to see a treament plan that doesn't result in meniscus removal, cause that only decreases career length for a short term improvement.

    Explains his shift from slightly more physical play in his younger years to flopping for more free throws and shooting more threes. Wonder how Chuck and Shaq would feel knowing he’s actually been playing through pain in his knee that most likely won’t ever go away. Love those guys but this really shifts the narrative a lot. Meniscus tears have always been the beginning of the end for so many players regardless of caliber and talent; and, if Joel played even 15 years earlier his career probably would’ve been over before he turned 28.

      Chuck and Shaq also played through injuries. That's why they feel entitled to talk, because they've lived it. If you remember watching basketball in the 90s like I do, you would remember that all of these guys were icing their knees on the bench. These aren't new issues but today's players are just way softer. Everyone flops, travels, plays terrible lazy defense and whines like children. You can't take 50 million a year and put up bench numbers. He deserves every bit of criticism that's coming his way.

    Basketball player and 2 knee surgeries as well. He came back too early. When I came back 6 months after a acl+ mcl surgery my knee would swell up and it felt really unstable. Did anyone see how out of shape he looks? His trainer should have made him swim to keep cardio up and it keeps the leg from atrophying. He should have never played in the Olympics, that's when he should have done PT every day. Anyone recovering from this surgery "Knees over toes guy" is one of the better youtubers to help you strengthen the surrounding muscles to prevent damage/ inflammation. 76er fans....take the season off with Joel.

    I'm a 6er fan and I saw this coming a mile away, some of our fans dont understand pattern recognition, but the dude hasn't been able to string 20 straight games together in 3 seasons. I just wish the front office was more honest to the paying fans. I think it's time to focus more on the healthy guys like Maxey. I'm cool with shutting Embiid down, get em as good as possible and then maybe focus more on defense n rebounds and let Maxey do more of the scoring. If Embiid can still be a 20ppg guy and get 13 rebs we'll be ok, but if we keep trying to run the offense through him, it just won't work, we cant rely on him.

    Sixers fan here, I’ve seen him have some small injuries here, and there as well, like hyperextensions, and then he’s out 3-4 games, I think the repeated injuries to both sides of his leg, took a toll on him

    The problem with him is he always rushed back to playing vs actually fixing his knee the first time. If he spent time and money the first time to do a procedure to clean up his ish, stem cell treatment for a year while dropping weight he might have had a chance.

    I would not wish meniscus injuries on anyone. I completely tore my lateral meniscus in my left knee in 2018 (doc said it looked like a bomb went off in it) and it pretty much ended my football playing days. Somehow my ligaments stayed intact. I tried to continue playing in college after the repair and my knee just never felt right. I had a secondary surgery in 2019 for a partial meniscectomy to remove a part that didn’t heal properly. And during surgery the pictures of my cartilage looked terrible, completely frayed like an old rope. Imagine being 18 and told that you’re showing early signs of arthritis. Depression.

    Stuff like this makes me appreciate that Chris Paul had his lateral meniscus in his right knee removed in 2010. I always kinda expected that to derail his career at some point, and it never did. His size definitely helped, but still felt unlikely.
    I think I remember reading that players sometimes choose to get parts of their meniscus removed rather than repaired because the recovery time is much shorter, as well? I'm not an expert so I don't know how true that is, but could be another factor pushing players toward bad long-term outcomes, in hopes of making it back for the playoffs.

    I had two arthroscopic surgeries on the same thing. Outer meniscus, left knee, baseball injury. I'd be running and my knee would completely give out and I'd slam into the ground. It no longer gives out, but if I stand on it too long, it gets pretty sore.

It’s sheer will and pain tolerance getting him through NBA games. The average citizen couldn’t play at LA fitness for 40 minutes with his knee let alone 40 in the NBA.

    Look, I'm anti load management essentially, but I don't think the average person even remotely understands how serious, painful, and debilitating some of these injuries may be. Let alone, I assume most of these people making these comments have never played sports at a high level regardless nor played through pain.
    That being said, there are so many athletes who had chronic knee issues and had to retire or fell off basically in their prime years. Todd Gurley is a prime example, he was cooked by like age 24-25 after being the best RB in the NFL. D-Wade had a long career, but his knees were fried. D-Rose as well. There are endless similar situations. And when you factor in a player of Embiid's size, knees and ligaments/joints in general are nothing to play with. He isn't some 5'8 170lb guy people.

    The only major sin that I can see if they didn’t force him to undergo a meniscus repair rather than a meniscectomy in 2017. They weren’t championship contenders back then and would have had the time to wait for the full recovery.
    But then again it might have been impossible for this case.

    Highly recommend Sihvonen’s paper in NEJM about partial meniscectomy. Seems to indicate that the procedure isn’t really effective despite being standard of care a lot of places. Meniscal repair or replacement seem to have actual clinical benefits that removal doesn’t.

      "The Process" was just an excuse for ownership not to have a payroll while they sold "hope" and "the future" to its fanbase for a few seasons. The initial tank made sense for them to do but they milked it for years and made all kinds of bad decisions along the way because the ownership doesn't actually understand the nuances of or care about basketball. They only own the team because they want to build a new stadium near Philly's Chinatown district which nobody in the city actually wants. Every other sports stadium has been located in a huge parking lot in South Philly for half a century but they have this brilliant idea to gentrify an area of the city while also exacerbating already heavy city traffic near there. Then they'll sell the team at its increased value from the new stadium. The owner of the team also owns the Washington Commanders (Rivals to the Eagles) and the New Jersey Devils (rivals to the Flyers). All billionaire sports team owners are ruthless profiteers but the Sixers/Commanders/Devils owner might just be the biggest carpetbagger in all of sports.

    If you don't want the expectations, then don't take the money. Sounds callous but that's what comes with these massive super-max or max contacts: a lot of expectations for on-court production. They're not paying you that much to put up 15 ppg or sit out half the season. And if that's truly what you're able to contribute to a team, then don't expect anywhere near that amount on your next contract.

    I tore part of my meniscus about 10 years ago. My swelling was coming down as I approached surgery. I skipped the surgery. I get around fine now. My knee stopped clicking after a while. I run, hike, no issues. I think I made the right choice. But I also don’t play in the NBA.

    I suffered a miniscus injury last summer. And Im telling you, it's not easily managed. Sometimes you feel great. But if that flap, created from that tear, starts flipping around. Ouch!! I literally just got surgery for a partial miniscus removal last week. Obviously Im not a pro athlete, but I am a huge basketball fan and I played basketball on a decently high level on this injury. It really is a tough one.

    You also have to keep in mind that Joel embiid was injured in college. It wasn't a knee injury. It was a broken bone in his right foot but when you're that big a dude that's probably going to always bother you.

    TLDR, they CHOOSE TO COME BACK FAST AF, so they did a MENISCECTOMY INSTEAD of allowing it time to heal and rehab, AND NOW HE HAS TO LITTLE LEFT OF HIS LATERAL MENISCUS, therefore HE WILL ALWAYS HAVE KNEE PROBLEMS ala Lonzo Ball

    I had a meniscus knee injury 5 years ago. I couldn't run on it. I avoided surgery and I did rubber band exercises and hit the chiropractor and sauna steam bath ice bath hot tub ocean everyday. After 1 month my meniscus healed enough to cushion my knee and run hard playing sports. Lesson is never do surgery
