This can easily be explained First: a blackflash happens in less than a hundredth of a second, as we saw with mahito's 0.2 domain expansion sukuna doesn't have the time to react to that and cast enchain in order to switch with yuji Second sukuna probably though that yuji could beat mahito even in a weakened state so he wouldn't have prepared for that And finally sukuna had already possessed yuji earlier that night and his body had just recently re adapted to sukuna so trying to do it again would be more difficult

Probably awakening sooner and slowly becoming more like Sukuna, doing anything to kill Mahito.

Like for example using idle transfig to merge Todo onto himself like some grotesque siamese twin or something, allowing him to keep him close at all times so it's a lot harder to destroy such a valuable asset. Or maybe he could hold onto a bunch of transfigured sorcerers as backup supplies of cursed energy, basically consuming them as needed to siphon their cursed energy for a boost. Or maybe even grafting them onto himself with their increased negativity allowing them to produce a constantly flux of additional CE to bolster himself. I imagine him wearing them embedded into his body at various points like jewels or decorations, both as a testament to his evil -showing of the people he's ruined- and as a means to enhance himself further.
Even if she dies, Yuta just needs to eat a part of her corpse and Mahito is fucked. That's if we don't keep in mind that with Yuki present Yuta might end up with the same capabilities as Yuji, since he is able to curse a soul and likely has a subconscious understanding because of it.

How I actually imagined it going as well, with minor tweaks of Sukuna intervening towards the end, or trying to but Yuji suppressing him making him either use the Binding vow the 2 made, or letting it go knowing he has a chance to incarnate in another vessel by probability.

I could see Mahito sending out goons to fight sorcerers. Leading to 1v1 fights for the main cast. Or finding the vessels Kenjacku left by chance. Adding Reincarnated sorcerers to the mix. So, kind of confusing for me, not really this what if, maybe a little, but more so the Yuji BM one, wasn't the durability of Mahito's true form because of a binding vow he made where he wasn't able to transfigure himself except for his elbow blades in exchange for 200% durability, and quicker extension of the elbow blades or something along those lines? What I'm saying is, shouldn't the added stuff to his true form count as transfiguring it?
I should have made this a bit more clear but he basically made a new true form that was different from the original. The BV itself was also less effective but Mahito made up for that with CE amount, experimentation and forcing himself to train harder
