The thing about doctors wearing unusual capes and masks back in the days comes from the Medieval Ages. Apparently, there was a place in the old Italy, called Poveglia Island where all Italian people who either suffered from a physical handicap, prisoners, witches or even people who were touted to be cursed were transported to this Island by ship, tied up and burnt alive until ashes so "the Devil will leave their souls". Nowadays the place is one of the most haunted places on Earth, and people who by the chance had to bring the "sacrificed people" to their death, were complaining about smell and had to wear ugly masks cause they thought that by wearing them, the Devil won't recognise them as humans but as devils. Also they were filling their masks with planta and flowers due to the insane smell from the human ashes.
