I learned this when I was in transcription over 20 years ago. I remember my instructor even saying Doctors really should take a class on properly dictating. You would hear them eating, or chewing gum or rustling papers while they rush through dictating the report, lol. It made it difficult to catch everything sometimes and you would have to ask, that was kind of rough for me!
Interesting insight! Lol I remember learning to know how to query the doctor because it's a major thing to be on point with documentation. I am brushing up my skills to get a job and get certified. Thank you!
Here in Australia we are called medical records clerks, and we are not required to code the records. A coder is at a higher rate of pay. We are paid 35 dollars an hour as clerks.
As a student coder, I keep coming back to wondering how long does it take to read the chart "cover to cover"? I keep seeing that coders are required to code so many charts a day it sounds like there is less than 7 minutes per chart in many settings.
What you will learn Donna Corbett, is how to recognize forms that have information that you need to code. Once you learn your facility chart, you will know exactly what to look for to code and you will be able to do it faster and faster.

I ALSO HAD COUNTLESS ONE ON ONE SESSIONS WITH PSYCHOLOGIST... I WENT TO THE MEDICAL RECORDS OFFICE WHEN I WAS 19 AND THEY GAVE ME THIS PIECE OF PAPER WITH MULTIPLE NUMBERS AND LETTERS THEY TOLD ME NOT TO LOSS IT BECAUSE THAT WAS MY MEDICAL RECORDS WELL I LOST IT AND NOW THERE TELLING ME THAT MY MEDICAL RECORDS ARE LOSS😲 IS THERE ANYWAY I CAN GET ALL MY MEDICAL RECORDS BACK??? OMG Tina Hyde. I know exactly what you are talking about. We had physicians using the bathroom and flushing the commode while dictating.

Can a medical receptionist enter diagnostic and procedural codes into the EMR?
What would you code first if you had a diagnosis and a procedure done in one outpatient visit? Procedure code or diagnosis code?
It would depend on what you are submitting the codes for mariachilove93. Software? School? Test?
