could a service user raise their leg up like recovery position ...discomfort ..merely cross over one foot over the over gently

Why do u need for feet ? better not to use pillow if u r talking about airway
no need to roll him over. slide sheets can be pulled down from the head to the feet. I'd go mental if they did this to my relative
