Homes not hospitals

NHS England commissions the Small Supports Programme nationally, for local systems to be able to benefit from this innovative and bespoke model of care. The purpose of the programme is to provide support to people with a learning disability and autistic people who have experienced long stays in mental health hospital, through a service designed around the person, enabling them to live a healthy and fulfilling life outside of a hospital setting.
This film tells the story of Mark, who was supported by Unique Support Solutions to return to community to live a rich and fulfilling life after many years in hospital.
