After reading metamorphosis i realized the apple could be a reference to book. Spoiler about book After few weeks of transformation Gregor Samsa's mom wants to clean his room with his sister. Samsa knew about her health issues and seeing him like this would hurt her so he just hid under couch. After some time when his mom and sister went to take a break he went out to save the portrait on the wall from getting thrown out like other objects. His mom saw his new physical form and she fainted. While his sister trying to get something to help her mother he wanted to wait near even tho he can't help. His father came in and when he saw Samsa out of his room he thought Samsa hurt them intentionally. So he started to chase him like trying to scare him. Since he couldnt able to catch samsa he started to throw "apples" Which ends up one is stuck inside his back and limits his moving capacity with pain and inflammation. They realized it wasn't needed and since they dont have enough courage to get that apple back he wasnt able to heal and had to live with that apple till he dies. So we knew "apple" symbolizes mafuyu but it could also be a reference like how they are taking care of pain caused by apple. Like i said samsa's family didn't take off that apple from his back and he had live with it but we can see mizuki (comforting pain) kanade (holding pain close to her) and ena (trying to reach that pain) Especially ena makes most sense to me like she wants to reach and take off that apple to free mafuyu from her pain
