    The seated hamstring curl with the resistance band shown at the 7:50 mark, can I do that from the first day of surgery? I am having a hamstring graft used for my acl reconstruction surgery. I am due surgery next week. How soon do I start these rehabilitation exercises? As soon as I wake up from surgery? Like on the same day?

      Day 1 it will probably be more of a non resisted heel slide on the table or on the floor but you should be able to progress fairly quickly into resistance. Always useful to have a guide in person or online who treats the issue regularly to help guide you with these decisions. Happy to help, if you don't have anyone locally.

    What is the device you are using for knee extension with the weight plate suspended between two straps? Is it available for purchase or is it just something you use in your clinic?

    I'm 10 weeks post op for a tibial plateau fracture type IV repaired with ORIF, complicated by a femural fracture which was repaired via femural nail. I'm still really struggling with achieving full extension. I'm doing a weighted knee hang 4x per day but it feels like I'm now hitting a hard stop.....

      This is definitely more complicated of a scenario. How's healing been, they saying that bone is healing nicely around the ORIF and femur? I would expect this to be more drawn out than an ACL.

      @@sportsrehabexperts yeah, bone is healing nicely, although still only partial weight bearing. This is limiting the activities available to help with the extension... Just becoming very frustrating, especially reading that obtaining extension can be time limited .. and with the current vivid situation, I'm lucky if I see the PT every 3 weeks

      @@TheBeanyi yea the weight bearing status is likely limiting your progress. But I'm sure the surgeon has reasons for that and you should abide by them 100%! Or have a conversation directly with him. Something useful to maybe consider would be "BFR Training" as a way to keep building quad and hamstring strength during this time.

      @@sportsrehabexperts I'll definitely have a look, thanks so much. My flexion is almost sufficient to allow cycling which will be a big bonus when it comes to building back my quad and hamstring strength.

    Can I do these before the surgery? Because I have been doing things like this without external weight

      Yes, before and after surgery these are good things to be doing as you are physically able

    February 28 tore my acl n miniscus in a game had my surgery March 19 still don’t have full range of motion n a lot of scar tissue. Recently did a manipulation to remove it still tight

      Sorry to hear that Karu, likely it will come down to frequency and load of doing the correct exercises. First, I would ask have you put forth a full and persistent effort yourself? Not bashing you, just have not context of your scenario and its important that you work on this often and consistently to make progress. If you are doing everything that is asked of you, most general PTs give out general exercises that are not very challenging and give you general results. Very little parameters for specificity unfortunately. The longer you lack extension the more adamant you will have to be long term to get it back as well.

    I'm going close to 10 weeks post op. I can touch the back of my knee to the floor with my post op acl knee,but I still can't raise my heel while touching the knee to the floor (like my other knee). I'm getting worried that by now I'll never get full extension back

      @@sportsrehabexperts sorry for the late reply. I've been massaging, stretching, patellar massages. I tried doing terminal knee extensions but my knee locks on me. Same as squats,can't fully extend when I come back up.

      @@OneSmellyTaco yea locking sensations are not something you want to work through. Have you done any open chain knee extension exercises such as short arc quads or knee extension with resistance? If not painful, may want to consider that. If it hurts or does not improve, I would need to take a look at it online to get a better idea/recommendation.

    How should we sleep post surgery? Can we sleep with a pillow under our knee/hamstring and then stretch it out during the day via low load gravity knee extension? What is the best way to sleep in order not to further mess-up mobility

      I would avoid the pillow as much as possible/tolerable even when sleeping and try to let the knee stretch out during the night too.

      @@TopBoi93 If you can tolerate it the above option would be the ideal! But many people have trouble sleeping that way as its not very comfortable. Try starting that way, then do what you need to do to get some sleep. But avoid putting the pillow under the knee to flex it greatly.

    I am 5 months post aclr surgery and still can’t achieve back my normal hyperextension, is there anything I can do? I really would like some help.

      Not sure how much hyperextension you are lacking or how much you currently have? Hyperextension takes more diligence and time then getting the leg to 0 degrees

    Is there a point after surgery where you can’t achieve full extension? I’m 5 weeks post op and I still can’t fully extend

      Takes a lot of diligent work. Frequency of doing it multiple times a day 2-4x per day for 10-20min if you are really taking it seriously. But also 5 weeks out isn't terrible, the last couple degrees can be a bit stubborn and take longer but also requires more persistence.

      @@sportsrehabexperts I had to wait 14 months before my acl surgery, and during prehab I tore my meniscus too, so I had a lot of muscle atrophy. I can now fully extend while doing the prone hang but I can’t when I’m standing or trying to straight leg raise. Is this because of the muscle waste?

    My surgery happened 7 months ago. My knee bending is fine, but I cannot fully straighten it. Only about 90% extension is possible. I feel scar tissue stiffness, and I am scared now. Will I ever fully recover?"😢sir?

      Well I would make sure you are measuring it correctly. You dont have to be perfectly symmetrical to the other side. If you can get it to 0 deg or fully straight then that is sold enough to be functional without pain. Some hyperextension 2-3 degree is ideal. Pain and range of motion dont correlate 100% but having the extension is definitely ideal

    Sir, i had an ACL surgery on 2019, its been 29months . Can i do sprint running now. Should i do weight training first and then start running? When i normally run i feel little discomfort on my knee. What should i do sir?

      Impossible to answer that question for a simple TH-cam comment. This is why I have a career and charge $$ for my service. Because it takes progression and preparedness to come back from an injury. Time is only one component and has very little impact on my decision to say an exercise is useful or perhaps unsafe. The only bad exercise is the one you have not prepared for.

    It’s been a fucking year in a half since my acl surgery and I still don’t have my knee extension back which is causing me to miss games during the football season how consistent should I do these to get back ASAP

      Sorry to hear that its still an issue for you Chris. Unfortunately if extension is not restored within the first 12 weeks it becomes very hard to improve it. Not saying that it can't be improved, but it will be take persistence and likely doing these things multiple times a day for months if it were to improve. If you can't get the knee to neutral this is an issue! If you are missing 2 degree or so of hyperextension compared to the other side, not ideal but not a deal breaker either.     Now after surgery,I am feeling same problem

      Sorry to hear the continued struggles! It can be a challenge restoring motion after ACL surgery. Extension you really want to emphasize stretching it the first 3 months pretty aggressively. Because after the 3 month mark it becomes harder to get extension to make continued improvement. That being said, progress can still be made and be expected over a 1 year timeline after surgery if you are diligent about it. It isn't the most comfortable thing in the world to mobilize, but need to work through some discomfort likely after surgery. But be sure to consult with your local therapist about what is safe to be doing! I consult with people online as well if you'd like another opinion.

      unfortunately Artist, this is not enough information for me to make a safe recommendation. You should consult with your physician and rehab clinician locally who you trust to see what they recommend. In general, 29 months out is a long time post op. You should have already been doing A LOT of proper weight training to this point in order to prepare the body to run.
